基于BI-RADS超声风险分层系统的乳腺图像特征与腋窝淋巴结转移的相关性研究OACSTPCD
To Investigate the Correlation between Mammary Image Features and Axillary Lymph Node Metastasis Based on BI-RADS Ultrasound Risk Stratification System
目的:探究基于美国放射学会(ACR)乳腺影像报告与数据系统(BI-RADS)的乳腺图像特征与腋窝淋巴结转移(ALNM)的相关性.方法:选取2020年3月至2023年1月期间本院收治的165例乳腺癌(BC)患者171个乳腺病灶作为研究对象,所有患者进行乳腺超声(US)检查,分析BC的超声图像特征与ALNM的相关性.结果:BC患者ALNM与钙化灶有无存在显著相关性(P<0.05),与肿瘤单发/多发或弥漫、血流丰富程度、病灶最大径存在极显著相关性(P<0.001),与形态、边缘、纵横比、后方回声无显著相关性(P>0.05);将单个超声图像特征与ALNM相关的4个指标作多因素分析,钙化灶、血流丰富程度、肿瘤数量、最大径的相对危险度分别为:0.943、8.216、2.768、3.867,其中血流丰富度对于判断ALNM的意义较大;具有4个或5个恶性超声图像特征的乳腺病灶的ALNM阳性率较高.结论:基于BI-RADS超声风险分层系统,可以根据不同的图像特征判定腋窝淋巴结是否转移,为临床分期和评估预后方案的制定提供可靠依据.
Objective:To investigate the correlation between mammary image features and axillary lymph node metastasis based on BI-RADS ultrasound risk stratification system.Methods:171 breast lesions from 165 breast cancer patients admitted to our hospital from March 2020 to January 2023 were selected as the research subjects,and all patients underwent breast ultrasound(US)examination to analyze the correlation between ultrasound image features of breast cancer and axillary lymph node metastasis(ALNM).Results:ALNM was significantly correlated with calcification in breast cancer patients(P<0.05),and was significantly correlated with single/multiple or diffuse tumors,blood flow richness and maximum diameter of lesions(P<0.001),and no significant correlation with morphology,margins,aspect ratio,and posterior echo(P>0.05).The four indexes related to ALNM were multivariate analyzed,and the relative risks of calcification,blood flow richness,tumor number and maximum diameter were 0.943,8.216,2.768 and 3.867,respectively,among which blood flow richness was of great significance forjudging ALNM.Breast lesions with 4 or 5 malignant ultrasound image features have a higher ALNM positivity rate.Conclusions:Based on the BI-RADS ultrasound risk stratification system,whether axillary lymph nodes are metastasized can be determined according to different image features,which provides a reliable basis for the formulation of clinical staging and prognosis assessment.
