头颈肿瘤免疫治疗病人居家自我管理体验的质性研究
Home self-management experience of patients undergoing immunotherapy for head and neck tumors:a qualitative study

Abstract
Objective:To explore the self-management experience of patients with head and neck tumor immunotherapy at home,and to provide reference for nursing intervention and management.Methods:Objective sampling method was used and a total of 18 patients with head and neck tumor who received immunotherapy in the oncology department were selected in a Grade-A hospital in Changsha city from February to June 2023.Face-to-face semi-structured interviews were conducted,and data were analyzed by Colaizzi 7-step analysis method.Results:The home self-management experience of head and neck tumor immunotherapy patients was summarized into 4 themes and 11 subthemes:Deficiency of home self-management ability(limited cognition,insufficient skills,weak behavioral compliance),lack of home self-management support resources(insufficient family support,lack of professional guidance),heavy psychological burden of home self-management(uncertain prognosis of side effects,disorganized self-image,increased stigma),behavioral motivation of home self-management(peer support and medical treatment)Staff support,a sense of responsibility for personal health and family,a sense of faith in active treatment to overcome illness).Conclusions:During the period of head and neck tumor immunotherapy patients at home,nursing staff should pay attention to patients'psychological feelings,physiological experience and support needs,provide more targeted guidance and intervention,improve their self-management ability,and improve their quality of life.关键词
