A correlation study on the progression of adult laryngeal papilloma disease with IL-17 and IL-1β

目的 探究白细胞介素17(interleukin-17,IL-17)及IL-1β与成人喉乳头状瘤疾病进展及人乳头状瘤病毒(human papilloma virus,HPV)感染状态的相关性.方法 收集2010年1月~2020年12月首都医科大学附属北京同仁医院耳鼻咽喉头颈科手术治疗的成人喉乳头状瘤患者的组织样本,将患者分为乳头状瘤组和疾病进展组.对样本行RNA原位杂交判断HPV分型,免疫荧光检测IL-17及IL-1β表达,分析其表达的组间差异及与HPV高危亚型的相关性.结果 乳头状瘤组及疾病进展组间IL-17及IL-1β免疫荧光阳性率存在显著差异,疾病进展组中IL-17及IL-1β阳性率显著升高(t=5.460,P<0.001;t=10.030,P<0.001).HPV高危亚型与IL-17及IL-1β表达相关,HPV高危型患者中IL-17及IL-1β表达水平显著升高,其中,IL-17表达差异具有统计学意义(t=4.554,P=0.0004).结论 IL-17及IL-1β与成人喉乳头状瘤疾病进展及HPV高危型感染状态有着密切相关性,HPV高危型加剧组织局部IL-17及IL-1β聚集,造成局部免疫微环境改变,进而引发疾病进展.

OBJECTIVE To explore relationship between the progression of adult laryngeal papilloma disease and HPV infection status with IL-17 and IL-1β.METHODS Tissue samples were collected from adult patients with laryngeal papilloma who underwent surgical treatment at the Department of Otolaryngology Head and Neck of Beijing Tongren Hospital from January 2010 to December 2020.We divide patients into papilloma group and disease progression group.RNA in situ hybridization was performed to determine HPV typing,and immunofluorescence was used to detect IL-17 and IL-1β expression,analysis the differences in IL-17 and IL-1β expression and correlation with high-risk HPV type.RESULTS There were significant differences in the positivity rates of IL-17 and IL-1β immunofluorescence between the papilloma group and the disease progression group.The positivity rates of IL-17 and IL-1β were significantly increased in the disease progression group(t=5.460,P<0.001;t=10.030,P<0.001).The high-risk type of HPV is correlated with the expression of IL-17 and IL-1β.The expression levels of IL-17 and IL-1β are significantly increased in HPV high-risk type patients,and the difference in IL-17 expression is statistically significant(t=4.554,P=0.0004).CONCLUSION There is a close correlation between the progression of adult laryngeal papilloma disease and IL-17 and IL-1β.High risk HPV exacerbates local expression of IL-17 and IL-1β in tissues,then causes changes in the local immune microenvironment and leading to disease progression.
