贝叶斯法精准调整华法林剂量的案例报告1例及文献分析
A CASEREPORT OF PRECISE DOSE ADJUSTMENT OF WARFARIN USING BAYESIAN METHOD AND LITERATURE REVIEW

Abstract
Objective To explore the effect of clinical pharmacists applying Bayesian method to adjust the medication management of warfarin in hospitalized patients and review the literature.Methods The case of a patient with atrial fibrillation and pulmonary embolism treated with warfarin dose adjustment by Bayesian method was analyzed,and the related literature at home and abroad was reviewed and analyzed.Results In this study,a clinical pharmacist participated in the drug treatment of a patient with atrial fibrillation and pulmonary embolism,and the Bayesian method was used to adjust the dose of warfarin.The Bayesian method was used to adjust the dose of warfarin in patients taking warfarin,and the dose was evaluated individually,and the risk of thrombosis and bleeding events was reduced.The Bayesian method uses the maximum a posterior-estimation method to predict the dose of warfarin,which can accurately guide individualized medication and can be used in clinical practice.Con-clusions Using the Bayesian method to predict and adjust the dose of warfarin,to give patients individualized medication guid-ance,to achieve homogeneous pharmaceutical care inside and o utside the hospital,and to explore a new model of pharmaceutical care by clinical pharmacists.关键词
