近红外荧光成像在腹腔镜检测隐匿性子宫内膜异位症病变手术中的应用
Near-infrared fluorescence imaging for detection of occult endometriosis:A pilot study

Abstract
Objective To evaluate the diagnostic utility of near-infrared fluorescence(NIR)imaging in detect-ing occult endometriosis lesions in conjunction with conventional white-light laparoscopy(WL),thereby exploring its potential as a diagnostic adjunct in endometriosis management.Methods Over a one-year period from February 2021 to February 2022,twelve patients diagnosed with endometriosis at Gansu Maternal and Child Care Hospital underwent laparoscopic surgery.The surgical protocol involved initial assessment using WL to identify suspected lesions,followed by NIR imaging utilizing indocyanine green(ICG).All identified lesions were excised and subjected to histopathological analysis for confirmation.Results A total of 61 lesions were biopsied,including 49 suspected lesions and 12 control lesions.WL detected 28 suspected lesions,of which 24 were histopathologically confirmed positive.NIR detected 11 suspected lesions,with 10 confirmed positive upon histopathological examination.Concurrent application of WL and NIR led to the detection of 10 suspected lesions,with 9 subsequently confirmed positive by histopathology.The detection rates were calculat-ed as 76.7%(33/43)for WL and 44.2%(19/43)for NIR.The kappa coefficient for suspected lesions was 0.686,indicating substantial agreement,whereas for WL-detected lesions,it was 0.411,indicating moderate agreement.Statistical analysis revealed significant differences in sensitivity,accuracy,and negative predictive value between WL combined with NIR and WL alone(P<0.05).Conclusion NIR imaging technology cannot entirely supplant the current gold standard of WL imaging technology.It should be integrated with WL imag-ing to achieve complementary imaging modalities,thereby enhancing the identification and eradication of oc-cult endometriosis.关键词
