Abstract
Objective To investigate the effect of high-strength fiber periodontal splint combined with metronidazole film on the periodontal health status and tooth aesthetic satisfaction of elderly patients with moderate to severe periodontal disease.Methods Elderly patients with moderate to severe periodontal disease in the author's hospital from June 2019 to December 2021 were selected for a prospective randomized controlled study,according to different treatment methods,they were divided into study group(n=56)and control group(n=56).Both groups were given basic treatment,on the basis of which the control group was treated with nylon ligatures once a week at follow-up visits and metronidazole film,while the study group was treated with high-strength fibre periodontal splints combined with metronidazole film,and after treatment with high-strength fibre periodontal splints,metronidazole film was placed in the periodontal pocket once a week at follow-up visits.After treatment,followed up for 3 months after treatment,the efficacy,tooth aesthetic satisfaction and periodontal index[periodontal pocket depth(PD),sulcus bleeding index(SBI),plaque index(PLI)]before and 3 months after treatment,gingival crevicular fluid inflammation factor indexes[procalcitonin(PCT),IL-1β,TNF-α]levels,chewing function(chewing efficiency,bite force)and oral health quality of life score(GOHAI)of the elderly were compared between the two groups.Results Three months after treatment,the total effective rate of the study group(94.64％)was higher than that of the control group(82.14％)(P＜0.05).The levels of PD,SBI and PLI in both groups were lower than before treatment,and the study group was lower than the control group(P＜0.05).The levels of PCT,IL-1β and TNF-α in gingival crevicular fluid in both groups were lower than before treatment,and the levels in study group were lower than those in control group(P＜0.05).The masticatory efficiency and biting force of the study group were higher than those of the control group(P＜0.05).The GOHAI score of the study group was lower than that of the control group(P＜0.05).The satisfaction of dental aesthetics in the study group was higher than that in the control group(P＜0.05).Conclusion The combination of high-strength fiber periodontal splint and metronidazole film for the first time in the treatment of elderly patients with moderate and severe periodontal disease has a significant effect,which can effectively reduce the periodontal inflammation and improve the periodontal status,and enhance the chewing function,thereby further improving the quality of oral health and improving the satisfaction of dental aesthetics.关键词
