Study on the efffects of in situ rotation surgery on the dental body and periodontal supporting tissues of impact-ed anterior teeth in Begal dogs

目的 构造模拟原位旋转术治疗埋伏前牙的比格犬埋伏前牙原位旋转模型,研究原位旋转术对比格犬牙体及牙周支持组织的影响.方法 研究于2022年1月至2023年10月在口颌系统重建与再生全国重点实验室进行.选取12只雄性比格犬根据实施原位旋转术后观察时间(术后1、3、6个月)随机分为1月组、3月组和6月组,每组4只;随机选择口腔一侧上下颌切牙(共6颗)行原位旋转术(研究侧),对侧同名切牙行开窗不旋转(对照侧).比较6月组牙齿萌出后1、2、3周和1、3、6个月探诊深度(PPD),采用锥形束CT测量比较每组两侧根长、舌侧牙槽骨厚度、唇侧牙槽骨厚度、唇侧牙槽骨丧失高度,采用MicroCT测量比较每组两侧牙槽骨的骨体积分数(BV/TV)、骨表面积骨体积比(BS/BV)、骨小梁厚度(Tb.Th)、骨小梁分离度(Tb.Sp)、骨小梁数目(Tb.N)及牙周膜厚度,比较每组两侧牙齿强度(断裂载荷),并采用HE和Masson染色分析两侧牙体、牙周组织改变及计算胶原容积分数(CVF).结果 6月组研究侧和对照侧牙齿萌出后各时间点PPD测量结果比较,差异均无统计学意义(均P>0.05).锥形束CT测量:各时间组内研究侧与对照侧根长、舌侧牙槽骨厚度、唇侧牙槽骨厚度、唇侧牙槽骨丧失高度比较,差异均无统计学意义(均P>0.05).MicroCT测量:1月组研究侧BV/TV、BS/BV低于对照侧,差异有统计学意义(t值分别为8.237、2.979,均P<0.05);两侧Tb.N、Tb.Th、Tb.Sp及牙周膜厚度比较,差异均无统计学意义(均P>0.05).3月组、6月组两侧MicroCT测量项目比较,差异均无统计学意义(均P>0.05).牙齿强度检测:各时间组内两侧牙齿断裂载荷比较,差异无统计学意义(均P>0.05).HE和Masson染色:1月组研究侧较对照侧靠近牙根的牙髓腔内出现更多的血管分化、牙周韧带较薄甚至部分消失,研究侧CVF小于对照侧,差异有统计学意义(t=19.680,P<0.001);3月组和6月组研究侧与对照侧基本无差异.结论 原位旋转术治疗比格犬埋伏前牙效果较佳,长期观察结果显示其对牙体及牙周组织愈后未产生不良影响.

Objective To construct a simulated in situ rotation model for the treatment of impacted anterior teeth in Beagle dogs and investigate the effects of in situ rotation on the dental body and periodontal supporting tissues in Beagle dogs.Methods The study was con-ducted at the National Key Laboratory of Oral and Maxil-lofacial Reconstruction and Regeneration from January 2022 to October 2023.Twelve male Beagle dogs were randomly divided into the 1-month group,3-month group,and 6-month group based on the observation time after in situ rotation surgery(1,3,and 6 months postoperatively),with 4 dogs in each group.In one side of the oral cavity,in situ rotation surgery was performed on the corresponding incisor(6 teeth,study side),while on the contralateral side,a window was created without rotation(6 teeth,control side).The probing pocket depth(PPD)was compared at 1,2,and 3 weeks and 1,3,and 6 months after tooth eruption in the 6-month group.Cone beam CT(CBCT)was used to measure and compare the root length,lingual alveolar bone thickness,labial alveolar bone thickness,and labial alveolar bone loss height on both sides in each group.Micro-computed tomography(MicroCT)was used to measure and compare the bone volume fraction(BV/TV),bone surface area to bone volume ratio(BS/BV),trabecular thickness(Tb.Th),trabecular separation(Tb.Sp),trabecular number(Tb.N),and periodontal membrane thick-ness on both sides in each group.The tooth strength(fracture load)was compared between the two sides in each group.HE and Masson staining were used to analyze the changes in dental body and periodontal tissues on both sides and calcu-late the collagen volume fraction(CVF).Results There was no significant difference in PPD measurements at different time points after tooth eruption between the study side and the control side in the 6-month group(all P>0.05).CBCT measurements showed no significant differences in root length,lingual alveolar bone thickness,labial alveolar bone thick-ness,or labial alveolar bone loss height between the study side and the control side in each time group(all P>0.05).Mi-croCT measurements showed that the BV/TV and BS/BV in the study side of the 1-month group were lower than those in the control side,and the differences were statistically significant(t values were 8.237 and 2.979,respectively,both P<0.05).There were on statistical differences in Tb.N,Tb.Th,Tb.Sp or periodontal membrane thickness between the two sides(all P>0.05).There were no significant differences in MicroCT measurements between the study side and the con-trol side in the 3-month group and 6-month group(all P>0.05).There were no significant differences in fracture load in any time points between the two sides in each time group(all P>0.05).HE and Masson staining showed that there were more vascular differentiations in the pulp cavity near the root and thinner or partially disappeared periodontal liga-ment on the study side compared to the control side in the 1-month group,and the CVF on the study side was lower than that on the control side,the differences being statistically significant(t=19.680,P<0.001).There were no significant differences between the study side and the control side in the 3-month group and 6-month group.Conclusion In situ rotation surgery is effective in the treatment of impacted anterior teeth in Beagle dogs,and long-term observation results show that it does not have adverse effects on the dental body and periodontal tissues.
