目的:分析人工膝关节表面置换术患者术中发生低体温的相关影响因素.方法:采取随机抽样法抽取 2022 年 4-12 月于莆田学院附属医院行人工膝关节表面置换术患者 108 例为研究对象,观察并记录患者术中直肠体温变化情况,若术中直肠体温<36℃则判定为发生低体温.将术中发生低体温患者纳入发生组,未发生低体温患者纳入正常组.采用基线资料调查量表调查患者一般资料并比较,找出相关影响因素.结果:108 例人工膝关节表面置换术患者术中 45 例发生低体温,发生率为 41.67%;发生组≥75岁年龄段、手术室温度<21℃、术中输液量≥1 000 mL、手术时间≥2.5 h的患者占比均高于正常组,差异均有统计学意义(P<0.05);经logistic回归分析结果显示,年龄≥75岁、手术室温度<21℃、术中输液量≥1 000 mL、手术时间≥2.5 h是人工膝关节表面置换术患者术中低体温发生的影响因素(OR>1,P<0.05).结论:人工膝关节表面置换术患者术中低体温发生率较高,受到年龄、手术室温度、术中输液量及手术时间等因素的影响.

Objective:To analyze the relevant influencing factors of intraoperative hypothermia in patients undergoing artificial knee joint surface replacement surgery.Method:A random sampling method was used to select 108 patients who underwent knee joint surface replacement surgery in the Affiliated Hospital of Putian University from April to December 2022 as the research subjects.The changes in rectal temperature during the surgery were observed and recorded.If the rectal temperature during the surgery was less than 36℃,it was determined that hypothermia had occurred.Patients with intraoperative hypothermia were included in the occurrence group,while those without hypothermia were included in the normal group.Patients'general information were investigated and compared by using a baseline data survey scale to identify relevant influencing factors.Result:45 out of 108 patients undergoing artificial knee joint surface replacement experienced hypothermia during the surgery,with an incidence rate of 41.67%;the proportion of patients in the age group≥75 years old,operating room temperature<21℃,intraoperative infusion volume≥1 000 mL and surgery time≥2.5 hours in the occurrence group were higher than those in the normal group,the differences were statistically significant(P<0.05);the results of logistic regression analysis showed that age≥75 years,operating room temperature<21℃,intraoperative infusion volume≥1 000 mL and surgical time≥2.5 h were the influencing factors for intraoperative hypothermia in patients undergoing artificial knee joint surface replacement surgery(OR>1,P<0.05).Conclusion:The incidence of intraoperative hypothermia in patients undergoing artificial knee joint surface replacement is relatively high,which is influenced by factors such as age,operating room temperature,intraoperative infusion volume and surgical time.
