经颅磁刺激联合康复训练对帕金森病患者冻结步态功能的影响OA
Effect of transcranial magnetic stimulation combined with rehabilitation training on frozen gait function in patients with Parkinson's disease
目的 考察对帕金森病(PD)患者行经颅磁刺激联合康复训练后其冻结步态功能的变化.方法 回顾性分析徐州医科大学附属医院于2020 年3 月至2022 年6 月期间收治的 80 例PD患者的临床资料,根据护理方案的不同进行分组,将接受常规治疗联合康复训练的患者纳入对照组(40 例),将接受对照组干预方案联合经颅磁刺激的患者纳入观察组(40 例).对比2 组患者在干预前和干预 3 个月后的冻结步态功能、认知功能、生活质量以及平衡能力变化.结果 干预1 个月和3 个月后,2 组的步速均快于干预前,步长均大于干预前,起立-行走计时均短于干预前,且干预3 个月后的上述变化均优于干预1 个月后(P均<0.05);观察组在干预1 个月和3 个月后的上述变化均较同期对照组更优(P均<0.05).干预1 个月和3 个月后,2 组的Berg平衡量表评分、蒙特利尔认知评估量表评分和简易精神状态量表评分均高于干预前,PD患者生活质量自测问卷评分均低于干预前,且干预 3 个月后的上述量表评分均优于干预1 个月后(P均<0.05);观察组在干预 1 个月和 3 个月后的上述量表评分均较同期对照组更优(P均<0.05).结论 经颅磁刺激联合康复训练有助于改善PD患者的冻结步态功能和平衡能力,促进其生活质量和认知功能提升.
Objective To explore the effect of transcranial magnetic stimulation combined with rehabilitation training on frozen gait function in Parkinson's disease(PD)patients.Methods The clinical data of 80 PD patients admitted to the Affiliated Hospital of Xuzhou Medical University from March 2020 to June 2022 were retrospectively analyzed.They were grouped according to different nursing plans.Patients who received routine treatment combined with rehabilitation training were included in the control group(40 cases),while patients who received the intervention plan of the control group and the transcranial magnetic stimulation were included in the observation group(40 cases).The changes of frozen gait function,balance ability,quality of life,and cognitive function were compared between the two groups before intervention and after 3 months of intervention.Results After 1 and 3 months of intervention,both groups had faster walking speed,greater step length,and shorter Timed Up and Go Test time when compared with those before intervention,and the above changes after 3 months of intervention were better than those after 1 month of intervention(all P<0.05).The above changes in the observation group were superior to those in the control group after 1 and 3 months of intervention(all P<0.05).After 1 and 3 months of intervention,the scores of the Berg Balance Scale,the Montreal Cognitive Assessment,and the Mini-Mental State Examination in both groups were higher than those before intervention,and the score of the 39-item Parkinson's Disease Questionnaire was lower than that before intervention.The above scale scores after 3 months of intervention were better than those after1 month of intervention(all P<0.05).The above scale scores of the observation group were superior to those of the control group after 1 and 3 months of intervention(all P<0.05).Conclusion Transcranial magnetic stimulation combined with rehabilitation training can improve the frozen gait function,balance ability,and quality of life of PD patients,and promote the recovery of cognitive function.
