中药穴位贴敷、穴位埋线序贯疗法在2型糖尿病伴肥胖患者中的疗效观察OACSTPCD
Therapeutic effects of sequential therapy of acupoint application of traditional Chinese medicine and catgut implan-tation at acupoint on patients with type 2diabetes mellitus and obesity
目的:观察"中药穴位贴敷-穴位埋线"序贯疗法在2型糖尿病伴肥胖患者中的疗效.方法:选取2020年5月～2023年5月我院收治的102例2型糖尿病伴肥胖患者,随机分为对照组和观察组各51例,对照组给予常规治疗,观察组在对照组基础上给予序贯治疗,12周后比较治疗后两组患者临床疗效、中医证候积分、血糖水平[空腹血糖(FBG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1c)]、胰岛素抵抗指标[胰岛素抵抗指数(HOAM-IR)、胰岛素分泌指数(HOAM-β)]、肥胖指标[体重、体质量指数(BMI)、体脂率]、炎症因子[C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]及不良反应发生情况.结果:观察组临床总有效率高于对照组(P＜0.05).两组中医证候积分、FBG、2hPG、HbA1c、HOAM-IR、体重、BMI、体脂率及 CRP、IL-6、TNF-α 水平均降低,HOAM-β升高(P＜0.05),且观察组中医证候积分、FBG、2hPG、HbA1c、HOAM-IR、体重、BMI、体脂率及CRP、IL-6、TNF-α水平均低于对照组,HOAM-β高于对照组(P＜0.05).两组不良反应发生率比较无差异(P＞0.05).结论:"中药穴位贴敷-穴位埋线"序贯疗法在2型糖尿病伴肥胖患者中的效果明显,可有效控制血糖水平和体质量,改善胰岛功能和机体炎症状态,且患者依从性高、安全性较高,值得临床推广应用.
Objective:To observe the therapeutic effects of sequential therapy of acupoint application of traditional Chinese medicine acupoint and catgut implantation at acupoint on patients with type 2diabetes mellitus(T2DM)and obesity.Methods:A total of 102obese patients with T2DM admitted to the hospital from May 2020to May 2023were selected and randomly divided into control group and observation group,with 51cases in each group.The control group was given conventional treatment,while the observation group was given sequential therapy on the basis of the treatment of control group.The clinical efficacy,traditional Chi-nese medicine syndrome scores,blood glucose[fasting blood glucose(FBG),2-hour postprandial plasma glucose(2hPG)and glycosylated hemoglobin(HbA1c)],insulin resistance indices[insulin resistance index(HOAM-IR)and insulin secretion in-dex(HOAM-β)],obesity indices[body weight,body mass index(BMI)and body fat rate],inflammatory factors[C-reac-tive protein(CRP),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)]and the incidences of adverse reactions were compared between groups.Results:The total clinical effective rate in the observation group was higher than that in the con-trol group(P＜0.05).After 3months of treatment,traditional Chinese medicine syndrome scores,FBG,2hPG,HbA1c,HOAM-IR,body weight,BMI,body fat rate,the levels of CRP,IL-6and TNF-α in both groups were reduced,and HOAM-β was in-creased(P＜0.05).Meanwhile,traditional Chinese medicine syndrome scores,FBG,2hPG,HbA1c,HOAM-IR,body weight,BMI,body fat rate,the levels of CRP,IL-6and TNF-α in the observation group are lower,and HOAM-β was higher as com-pared with the control group(P＜0.05).The incidences of adverse reactions in the two groups were close(P＞0.05).Conclusion:The sequential therapy of acupoint application of traditional Chinese medicine acupoint and catgut implantation at acupoint is sig-nificantly effective for patients with T2DM and obesity.It can effectively control blood glucose level and body mass,improve pan-creas islet function and inflammatory state,with high safety.
