目的:分析单孔法电视辅助胸腔镜(uniportal video-assisted thoracoscopic surgery,UVTAS)肺叶切除术对肺结核患者免疫细胞水平、视觉模拟评分法(VAS)评分及并发症的影响.方法:选取2020 年 1 月—2022 年 1 月在内蒙古自治区第四医院接受UVTAS肺叶切除术的 100 例肺结核患者,随机分为对照组(n=50)和研究组(n=50).对照组接受双操作孔胸腔镜手术,研究组接受UVTAS肺叶切除术.比较两组免疫细胞水平、VAS评分及并发症的变化.结果:两组免疫细胞水平时间、组间、交互效应比较,差异均有统计学意义(P<0.05).术后 1 周,研究组CD3+T淋巴细胞、CD4+T淋巴细胞均显著高于对照组,CD8+T淋巴细胞低于对照组(P<0.05);术后 1 个月,两组免疫细胞水平,差异均无统计学意义(P>0.05).术后 1 周,两组VAS评分均比术后 1 d低(P<0.05),且研究组更低(P<0.05).研究组的术后并发症发生率显著低于对照组(P<0.05).结论:UVTAS肺叶切除术能够提高肺结核患者的免疫功能,减轻术后疼痛及并发症.

Objective:To analyze the effects of uniportal video-assisted thoracoscopic surgery(UVTAS)lobectomy on immune cell levels,visual analogue scale(VAS)scores and complications in patients with pulmonary tuberculosis.Method:A total of 100 pulmonary tuberculosis patients receiving UVTAS lobectomy in Inner Mongolia Fourth Hospital from January 2020 to January 2022 were selected and randomly divided into control group(n=50)and study group(n=50).The control group received thoracoscopic lobectomy with double operating holes,and the study group received UVTAS lobectomy.The immune cell levels,VAS scores and complications between the two groups were compared.Result:There were significant differences in immune cell level time,intergroup and interaction effect between the two groups(P<0.05).1 week after surgery,CD3+ T lymphocytes and CD4+ T lymphocytes in the study group were significantly higher than those in the control group,and CD8+T lymphocytes was lower than that in the control group(P<0.05);1 month after surgery,there were no significant differences in immune cell levels between the two groups(P>0.05).1 week after surgery,VAS scores in both groups were lower than those 1 day after surgery(P<0.05),and those in the study group were lower(P<0.05).The incidence of postoperative complications in the study group was significantly lower than that in the control group(P<0.05).Conclusion:UVTAS lobectomy can improve the immune function of patients with pulmonary tuberculosis and reduce postoperative pain and complications.
