Effects of Micro-implant Anchorage Correction on Soft Tissue Profile and Nasolabial Chin Coordination in Patients with Bimaxillary Protrusion

目的:探讨微种植体支抗矫治双颌前突的临床疗效及对患者软组织侧貌和鼻唇颏协调性的影响.方法:回顾性分析2017年4月-2020年11月笔者医院就诊的60例双颌前突患者的临床资料,将采用传统口外加强支抗的30例患者纳入B组,将采用微种植体支抗的31例患者纳入A组,比较两组矫治时间、头影测量项目及牙周炎性微环境变化.结果:A组拔牙间隙关闭时间、矫治时间均短于B组(P<0.05);治疗后,A组U1-Ll角大于B组,U1-NA角小于B组(P<0.05).治疗后,A组上、下唇突距(UL-E线、LL-E线)大于B组(P<0.05);A组颏唇沟深度、鼻唇角、颏沟倾角大于B组(P<0.05).治疗后6个月,A组龈沟出血指数、牙龈指数、牙菌斑指数水平低于B组(P<0.05);A组牙周组织白介素-6、基质金属蛋白酶-2/-9、肿瘤坏死因子-α水平低于B组(P<0.05).结论:微种植体支抗对双颌前突具有明显正畸作用,和传统口外加强支抗比较,患者颜面部外侧貌与美学标准更相符,对牙周环境的刺激较小,同时可在一定程度上缩短矫治时间.

Objective To explore the effects of micro-implant anchorage correction on soft tissue profile and nasolabial chin coordination in patients with bimaxillary protrusion.Methods A retrospective analysis was performed on the clinical data of 60 patients with bimaxillary protrusion in the hospital between April 2017 and November 2020,including 30 cases given traditional extraoral reinforcement anchorage in group B and 31 cases given microscrew implant anchorage in group A.The correction time,cephalometric measurement items and changes of periodontal inflammatory microenvironment in the two groups were compared.Results The turn-off time of extraction space and correction time in group A were shorter than those in group B(P<0.05).After treatment,angle of U1-Ll in group A was greater than that in group B,while angle of U1-NA was smaller than that in group B(P<0.05).After treatment,protruding distance of upper and lower lip(UL-E line and LL-E line)in group A was longer than that in group B(P<0.05).After treatment,depth of mentolabial sulcus,nasolabial angle and inclination of chin sulcus in group A were greater than those in group B(P<0.05).At 6 months after treatment,sulcus bleeding index,gingival index and plaque index in group A were lower than those in group B(P<0.05),and levels of interleukin-6(IL-6),matrix metalloproteinase-2/-9 and tumor necrosis factor-α in periodontal tissues were lower than those in group B(P<0.05).Conclusion Microscrew implant anchorage has obvious orthodontic effect on bimaxillary protrusion.Compared with traditional extraoral reinforcement anchorage,it has more consistent facial profile with aesthetic standards and less stimulation to periodontal environment,which can shorten correction time to a certain extent.
