中医针刺联合舌肌训练对脑卒中恢复期吞咽功能障碍患者的临床疗效观察
Clinical Observation of Traditional Chinese Medicine Acupuncture Combined with Tongue Muscle Training in the Treatment of Dysphagia during Stroke Recovery Period

Abstract
Objective:To explore the clinical effect of acupuncture combined with tongue muscle training on swallowing dysfunction(SD)in convalescent period of stroke.Methods:A total of 98 SD patients in the convalescent stage of stroke who were admitted to The Second People's Hospital of Nantong from January 2020 to December 2021 were selected and divided into 2 groups with 49 patients in each group according to the different rehabilitation treatment plans of the patients.The control group received lingual muscle,soft palate and pharynx function training,and the observation group was given the rehabilitation treatment program of traditional Chinese medicine acupuncture combined with tongue muscle training,and the treatment lasted for 4 weeks.After the rehabilitation treatment,the efficacy was evaluated,the neurotransmitter levels(serum SP,5-HT)of the patients were measured,the neurological deficits and swallowing conditions were compared,and the safety of the rehabilitation treatment program was observed.Results:The total effective rate of the observation group was 93.88%(46/49),higher than that of the control group(77.55%,38/49,P<0.05).The levels of 5-HT and SP in the two groups of SD patients increased after the treatment,and increased more in the observation group than in the control group(P<0.05).After the treatment,the NIHSS and SAA scores of SD patients in both groups decreased,and decreased more in the observation group than in the control group(P<0.05).The adverse reaction rates of the observation group and the control group were 18.37%(9/49)and 12.24%(6/49),and there was no significant difference between the two(P>0.05).Conclusion:Traditional Chinese medicine acupuncture combined with tongue muscle training can improve the curative effect of SD patients in the convalescent period of stroke,increase the secretion of neurotransmitters,and improve the neurological deficit and swallowing function,with high safety.关键词
