以消化道症状首发的系统性红斑狼疮误诊原因探析
Exploration of Causes of Misdiagnosis of Systemic Lupus Erythematosus with Gastrointestinal Symptoms as the Initial Manifestation

Abstract
Objective To analyze the causes of misdiagnosis of systemic lupus erythematosus(SLE)with gastrointes-tinal symptoms as the initial manifestation,and to summarize the measures to prevent misdiagnosis.Methods The clinical data of 3 patients with SLE who had been misdiagnosed from December 2013 to September 2023 were retrospectively analyzed.Results All the 3 patients presented with gastrointestinal symptoms as the initial manifestation.One patient was misdiagnosed with acute gastritis and intestinal obstruction,1 as infectious diarrhea,acute gastroenteritis,and gastrointestinal spasm,and 1 as acute enteritis.Misdiagnosis lasted from 7 to 30 d.After admission,the 3 patients were confirmed as having SLE after de-tailed physical examination,follow-up medical history,improvement of blood immunological indicators and renal puncture pa-thology examination,and symptoms were relieved after glucocorticoid therapy.The symptoms disappeared after 3 months of follow-up.Conclusion There are various clinical manifestations of SLE,and early SLE with gastrointestinal symptoms as the initial symptom is less likely to be diagnosed.Clinicians should be vigilant to avoid or reduce misdiagnosis and mistreatment.关键词
