洁净层流手术间开放性脊柱手术中无菌灯柄细菌检出情况及危险因素分析OACSTPCD
Bacteria detection and risk factors of sterile lamp handle during open spinal surgery in clean laminar flow operating room
目的 探讨洁净层流手术间开放性脊柱手术使用的无菌灯柄不同时段的细菌污染状况及危险因素,为及时更换灯柄及实施干预措施提供依据.方法 选取在洁净层流手术间行开放性脊柱手术使用的275个无菌灯柄为研究对象,使用无菌棉拭子对灯柄外表面进行取样,对不同时段灯柄的细菌检出情况及危险因素进行分析.结果 开放性脊柱手术使用的275个无菌灯柄中从手术3 h开始共有41个(14.91%)灯柄检出细菌;培养出53株细菌,其中大肠埃希菌17株(32.08%),所占比例最高.单因素及多因素Logistic回归分析表明,合并开放性外伤、手术时间≥5 h、术中出血量≥400 mL、术中使用无菌灯柄对灯次数≥10次、灯柄有明显的血液污染及手术间人员数量≥8名是导致灯柄检出细菌的危险因素(OR>1,P<0.05).结论 洁净层流手术间开放性脊柱手术使用的无菌灯柄细菌污染主要由大肠埃希菌引起,灯柄的细菌污染与开放性外伤、手术时间、出血量、使用无菌灯柄对灯次数、灯柄有明显血液污染及手术间人员数量等多种因素有关,建议手术时间超过3 h时应更换灯柄.
Objective To investigate the bacterial contamination status and risk factors of sterile lamp handle used during open spinal surgery in clean laminar flow operating room at different time periods,so as to provide basis for timely replacement of lamp handle and implementation of intervention measures.Methods A total of 275 sterile lamp handles used during open spinal surgery in clean laminar flow operating room were selected as the research objects,and the outer surface of the lamp handle was sampled with sterile cotton swab,and the bacteria detection and risk factors of the lamp handle at different time periods were analyzed.Results Among 275 sterile lamp handles used during open spinal surgery,41(14.91%)lamp handles detected bacteria from 3 hours after operation;and 53 strains of bacteria were cultured,among which 17 strains(32.08%)were Escherichia coli,accounting for the highest proportion.Univariate and multivariate Logistic regression analysis showed that complicated open trauma,operation time≥5 hours,intraoperative blood loss≥400 mL,intraoperative use of sterile lamp handle against the lamp≥10 times,obvious blood contamination of lamp handle and number of operating room personnel≥8 were the risk factors leading to the detection of bacteria in lamp handle(OR>1,P<0.05).Conclusion The bacterial contamination of sterile lamp handle used during open spinal surgery in clean laminar flow operating room is mainly caused by Escherichia coli.The bacterial contamination of the lamp handle is related to open trauma,operation time,amount of blood loss,number of sterile lamp handle used against the lamp,obvious blood contamination of lamp handle and number of operating room personnel,etc.It is suggested that the lamp handle should be replaced when the operation time is longer than 3 hours.
