Abstract
Objective:To analyze the clinical effect of electrocoagulation hemostasis surgery under nasal endoscope in the treatment of hypertensive epistaxis.Method:Fifty patients with hypertensive epistaxis admitted to the Otorhinolaryngology Department of Ninghua County General Hospital from October 2020 to October 2022 were retrospectively selected.They were divided into nasal endoscopy group and tamponade group according to different treatment methods,25 cases in each group.The tamponade group was given nasal tamponade treatment,and the nasal endoscope group was given electrocoagulation hemostasis surgery under nasal endoscope treatment.The clinical efficacy at 7 d after treatment,clinical indexes,pain degree at 7 d after treatment,vital signs indexes before treatment and 24 h after treatment,adverse reactions and recurrence rate were compared between the two groups.Result:The total effective rate of nasal endoscopy group(96.00%)was higher than that of tamponade group(72.00%),and the difference was statistically significant(P<0.05).The nasal ventilation time,nasal mucosa normalization time and bleeding cessation time in the nasal endoscopy group were shorter than those in the tamponade group,and the visual analog scale(VAS)score was lower than that in the tamponade group,the differences were statistically significant(P<0.05).After 24 h of treatment,diastolic blood pressure(DBP),systolic blood pressure(SBP),heart rate(HR)and respiratory rate(RR)in the tamponade group were increased,and DBP,SBP,HR and RR in the nasal endoscopy group were lower than those in the tamponade group,the differences were statistically significant(P<0.05).The incidence of adverse reactions in nasal endoscopy group(4.00%)was lower than that in tamponade group(32.00%),and the difference was statistically significant(P<0.05).The recurrence rate of nasal endoscopy group was 0(0/25),which was lower than 24.00%(6/25)of tamponade group,and the difference was statistically significant(P<0.05).Conclusion:Electrocoagulation hemostasis surgery under nasal endoscope can effectively improve the treatment efficiency of hypertensive epistaxis,reduce the amount of blood loss,promote the restoration of nasal mucosa to normal,reduce the impact on heart rate and blood pressure,and reduce the recurrence rate,with significant effect.关键词
