非小细胞肺癌患者血清中CYFRA21-1、NSE表达及临床意义
Expression and Clinical Significance of Serum CYFRA 21-1 and NSE in Patients with Non-small Cell Lung Cancer

Abstract
Objective To explore the expression and clinical significance of serum cytokeratin 19 fragment antigen 21-1(CYFRA 21-1)and neuron specific enolase(NSE)in non-small cell lung cancer(NSCLC).Method A total of 132 patients with NSCLC were enrolled NSCLC group,90 patients with benign lung disease and 90 healthy patients were enrolled benign lung disease group and healthy control group.The expression levels of serum CYFRA 21-1 and NSE in the three groups were detected by chemiluminescence method,and their relationship with clinicopathology of NSCLC patients was analyzed.According to prognosis after 2 years of follow-up,patients were divided into survival group and death group.The application value of serum CYFRA 21-1 and NSE in the diagnosis and prognosis evaluation of NSCLC was analyzed by receiver operating characteristic curves(ROC).Result Compared with benign lung disease group and healthy control group,the levels of serum CYFRA 21-1 and NSE were higher in NSCLC group(P<0.05).The area under ROC curve of serum CYFRA 21-1 for evaluating the occurrence of NSCLC was greater than that of NSE(0.792 vs 0.671,P<0.05).The difference in levels of serum CYFRA 21-1 and NSE among NSCLC patients with different genders and age was not statistically significant(P>0.05).The expression levels of serum CYFRA21-1 and NSE in patients with clinical staging at stageⅢ/Ⅳand lymph node metastasis were higher than those with stageⅠ/Ⅱand lymph node non-metastasis(P<0.05).The expression level of serum CYFRA 21-1 in patients with lung squamous cell carcinoma was higher than that with lung adenocarcinoma(P<0.05).After2 years of follow-up,there were4 patients lost to follow-up.The levels of serum CYFRA 21-1 and NSE of death group(34 cases)were higher than those of survival group(94 cases)(P<0.05).AUC of CYFRA 21-1 combined with NSE for predicting 2-year prognosis of NSCLC patients was 0.798,greater than that of single index(0.689,0.665,P<0.05).Conclusion The expressions of serum CYFRA 21-1 and NSE are up-regulated in patients with NSCLC.The evaluation value of CYFRA 21-1 for the occurrence of NSCLC is greater than that of NSE.The high expressions of serum CYFRA 21-1 and NSE indicate poor prognosis of NSCLC patients.关键词
