脑卒中康复期患者前庭功能减退情况及其影响因素分析
Analysis of Vestibular Deficiency and Its Influencing Factors in Patients with Stroke in Convalescent Stage

Abstract
Objective To explore vestibular deficiency and its influencing factors in patients with stroke in convales-cent stage.Methods A total of 327 patients with stroke in convalescent stage admitted from July 2021 to May 2023 were se-lected and divided into vestibular deficiency group and non-vestibular deficiency group according to whether vestibular defi-ciency was present at 3 months after treatment.The general data of two groups were collected,and the influencing factors of vestibular deficiency in patients with stroke in convalescent stage were analyzed by univariate analysis,Lasso regression analy-sis and Logistic regression equation.R language was used to draw a nomogram model to predict the risk factors of vestibular deficiency.Receiver operating characteristic(ROC)was used to evaluate the predictive efficiency of the nomogram.Results The incidence of vestibular deficiency was 29.66%(97/327)in patients with stroke in convalescent stage at 3 months after treatment.The results showed that age≥60 years,smoking history,the National Institutes of Health Stroke Scale(NIHSS)score＞4 on admission,combined hyperlipidemia,hypertension,cervical spondylosis and anxiety,and proximal stroke of posterior circulation were the risk factors for vestibular deficiency in patients with stroke in convalescent stage.Vestibular function exercise was a protective factor for vestibular deficiency in patients with stroke in convalescent stage(P＜0.01).The C-index of the Logistic regression model for predicting vestibular deficiency in patients with stroke in convalescent stage was 0.889,and the area under the ROC curve of vestibular deficiency in patients with stroke in convalescent stage was 0.889(95%CI:0.849,0.928).Conclusion The incidence of vestibular deficiency is high in patients with stroke in convalescent stage.Age,smoking history,NIHSS score on admission,combined hyperlipidemia,hypertension,cervical spondylosis and anxiety,stroke site,and vestibular function exercise are the influencing factors for vestibular deficiency in patients with stroke in convalescent stage.Therefore,targeted prevention and treatment measures can be formulated to further improve prognosis.关键词
