Abstract
Objective To investigate the risk factors for white matter lesions(WML)in patients with initial small artery occlusion(SAO)cerebral infarction.Methods A total of 150 patients with initial SAO cerebral infarction admitted to the neurology department of Bozhou People's Hospital from January 2019 to July 2022 were selected.Based on the severity of WML,patients were categorized into the none WML group(30 cases),mild WML group(52 cases),and moderate-to-severe WML group(68 cases).After a retrospective analysis of clinical data from the three groups,a multivariate Logistic regression analysis was employed to identify the risk factors for WML in patients with initial SAO cerebral infarction.ROC curve analysis was performed to assess the predictive value of related risk factors for WML in patients with initial SAO cerebral infarction.Results Significant differences in age,hypertension,uric acid levels,and National Institute of Health Stroke Scale(NIHSS)scores were observed among the three groups(χ2/F=6.350,12.600,6.630,5.485;P＜0.05).Multivariate Logistic regression analysis revealed that hypertension and age were risk factors for WML in patients with initial SAO cerebral infarction(OR=7.039,1.090;P＜0.01).ROC curves showed that the AUC of hypertension and age in predicting patients with initial SAO cerebral infarction combined with WML was 0.691 and 0.698,optimal cut-off value was 150/90 mmHg and 56.5 years,Youden indice was 0.383 and 0.389,sensitivity was 73.52%and 71.70%,specificity was 68.42%and 67.2%,respectively.Conclusion Hypertension and age are risk factors for WML in patients with initial SAO cerebral infarction combined with WML.Hypertension and age have certain value in predicting WML in patients with initial SAO cerebral infarction combined with WML.关键词
