目的 探讨颈动脉血管周围脂肪组织(PVAT)强化与易损斑块的计算机断层血管造影(CTA)特征之间的关系.方法 纳入 86 例 2022 年 7 月至 2023 年 8 月于浙江中医药大学附属第二医院行颈动脉CTA并延迟扫描的患者.根据临床及影像表现将患者分为有症状组42例和无症状组44例.分析两组患者颈动脉斑块的CTA特征(斑块类型、斑块表面形态、斑块厚度及斑块强化率)、颈动脉周围脂肪密度(PFD)和PVAT强化率的差异性,以及颈动脉斑块易损特征与PVAT强化率的相关性.结果 斑块强化率和PVAT强化率呈正相关,其中两者在延迟扫描的相关性强于常规动脉期(r=0.328、0.284,P<0.05).在有症状组内,延迟扫描时斑块强化率与PVAT强化率呈正相关(r=0.439,P<0.05).有症状组的斑块厚度、常规动脉期斑块强化率、延迟扫描斑块强化率、常规动脉期PVAT强化率以及延迟扫描PVAT强化率高于无症状组(Z=-2.207、-2.410、-2.376、-2.773、-3.352,P<0.05).两名观察者间的斑块特征评估结果具有较好的一致性.结论 CTA延迟扫描可提高对斑块密度与PFD测量的准确性,能在一定程度上弥补常规颈动脉CTA存在的不足;PVAT强化率与易损斑块强化率的相关性不仅对评估斑块易损性有重要提示意义,而且可能对缺血性脑卒中的发生有一定的预测和评估价值.

Objective To investigate the relationship between enhancement of perivascular adipose tissue(PVAT)and computed tomographic angiography(CTA)features of vulnerable plaques in carotid arteries.Methods A total of 86 patients who underwent carotid CTA with delayed scanning from July 2022 to August 2023 at the Second Affiliated Hospital of Zhejiang Chinese Medicine University were included.Patients were divided into symptomatic group(42 cases)and asymptomatic group(44 cases)based on clinical and imaging manifestations.Differences in CTA characteristics of carotid plaques(plaque type,plaque surface morphology,plaque thickness,and plaque enhancement rate),perivascular fat density(PFD),and PVAT enhancement rate between the two groups were analyzed,as well as the correlation between the characteristics of carotid vulnerable plaque and PVAT enhancement rate.Results Plaque enhancement rate was positively correlated with PVAT enhancement rate,with a stronger correlation at the delayed scan than at the conventional arterial phase(r=0.328,0.284;P<0.05).In the symptomatic group,plaque enhancement rate at the delayed scan was moderately correlated with PVAT enhancement rate(r=0.439,P<0.05).Plaque thickness,plaque enhancement rate at the conventional arterial phase,plaque enhancement rate at the delayed scan,PVAT enhancement rate at the conventional arterial phase,and PVAT enhancement rate at the delayed scan in the symptomatic group were higher than those in the asymptomatic group(Z=-2.207,-2.410,-2.376,-2.773,-3.352;P<0.05).There was good consistency between two observers in the assessment of plaque characteristics.Conclusion CTA delayed scanning can improve the accuracy of plaque density and PFD measurement,which may partially compensate for the shortcomings of conventional carotid CTA.The correlation between PVAT enhancement rate and vulnerable plaque enhancement rate not only provides important clues for the assessment of plaque vulnerability,but may also have predictive and evaluative value for the occurrence of ischemic stroke.
