目的 探讨老年急性心肌梗死(AMI)伴心力衰竭患者血清中Kruppel样因子 15(KLF15)、微小核糖核酸 128-3p(miR-128-3p)表达与心功能的关系.方法 纳入 2020 年 8 月至 2021 年 8 月期间襄阳市襄州区人民医院收治的老年AMI伴心力衰竭患者 112 例作为AMI伴心力衰竭组,同期选取一般资料相匹配且单纯AMI患者 120 例作为AMI组,所有患者采用超声心动图进行心功能检测,记录左心室收缩末期内径(LVESd)、左心室舒张末期内径(LVEDd)、左心室射血分数(LVEF);测定患者血清中脑钠肽(BNP)、肌酸激酶同工酶(CK-MB)、心肌肌钙蛋白I(cTnI)、KLF15、miR-128-3p水平;Pearson法分析老年AMI伴心力衰竭患者血清KLF15、miR-128-3p表达水平与BNP、CK-MB、cTnI、LVESd、LVEDd、LVEF相关性,并分析血清KLF15 与miR-128-3p的相关性.结果 与AMI组相比,AMI伴心力衰竭组患者miR-128-3p表达水平、BNP、CK-MB、cTnI、LVESd、LVEDd水平升高(t/Z=38.306、5.865、7.997、20.575、16.037、17.368,P<0.05),LVEF、KLF15 水平降低(t=24.072、47.301,P<0.05).AMI伴心力衰竭患者血清miR-128-3p水平随心功能分级升高而升高(F=322.866,P<0.05),KLF15 水平随心功能分级升高而降低(F=221.228,P<0.05);老年AMI伴心力衰竭患者血清miR-128-3p表达水平与BNP、CK-MB、cTnI呈正相关(r=0.667、0.525、0.617,P<0.05),与LVEF呈负相关(r=-0.537,P<0.05);血清KLF15 表达水平与BNP、CK-MB、cTnI、LVEDd呈负相关(r=-0.531、-0.574、-0.562、-0.416,P<0.05),与LVEF呈正相关(r=0.624,P<0.05).TargetScan网站预测miR-128-3p与KLF15 具有靶向结合位点,Pearson分析显示,老年AMI伴心力衰竭患者血清miR-128-3p与KLF15 呈负相关(r=-0.853,P<0.05).结论 老年AMI伴心力衰竭患者血清KLF15 降低,miR-128-3p水平升高,均与患者心功能指标具有相关性,可作为判断AMI伴心力衰竭患者心功能的标志物.

Objective To investigate the relationship between serum Kruppel-like factor 15(KLF15)and micro ribonucleic acid 128-3p(miR-128-3p)serum expression levels and the cardiac function in elderly patients with acute myocardial infarction(AMI)and heart failure.Methods From August 2020 to August 2021,112 elderly patients with AMI and heart failure admitted to Xiangzhou District People's Hospital,Xiangyang City were included as the AMI with heart failure group,and 120 patients with simple AMI with matched general data during the same period were included as the AMI group.All patients underwent echocardiography to detect cardiac function and record left ventricular end-systolic diameter(LVESd),left ventricular end-diastolic diameter(LVEDd),and left ventricular ejection fraction(LVEF).The levels of brain natriuretic peptide(BNP),creatine kinase isoenzyme(CK-MB),cardiac troponin I(cTnI),KLF15 and miR-128-3p in the serum of the patients were determined.Pearson correlation analysis was used to analyze the correlation between serum KLF15,miR-128-3p expression levels and the levels of BNP,CK-MB,cTnI,LVESd,LVEDd,and LVEF in elderly patients with AMI and heart failure,and to analyze the correlation between serum KLF15 and miR-128-3p.Results Compared with the AMI group,AMI with heart failure group had higher expression levels of miR-128-3p,BNP,CK-MB,cTnI,LVESd and LVEDd(t/Z=38.306,5.865,7.997,20.575,16.037,17.368;P<0.05),and lower levels of LVEF and KLF15(t=24.072,47.301;P<0.05).The level of serum mir-128-3p in AMI patients with heart failure increased with the increase of cardiac function grading(F=322.866,P<0.05),while the level of KLF15 decreased(F=221.228,P<0.05).The expression level of serum miR-128-3p in elderly patients with AMI and heart failure was positively correlated with BNP,CK-MB and cTnI(r=0.667,0.525,0.617;P<0.05),and negatively correlated with LVEF(r=-0.537,P<0.05).The expression level of serum KLF15 was negatively correlated with BNP,CK-MB,cTnI and LVEDd(r=-0.531,-0.574,-0.562,-0.416;P<0.05),and positively correlated with LVEF(r=0.624,P<0.05).The TargetScan website predicted that miR-128-3p had a target binding site for KLF15.Pearson analysis showed that serum miR-128-3p was negatively correlated with KLF15 in elderly patients with AMI and heart failure(r=-0.853,P<0.05).Conclusion Serum KLF15 level is decreased and miR-128-3p level is increased in elderly patients with AMI and heart failure,and both are correlated with patients'cardiac function indexes,which can be used as biomarkers for judging the cardiac function of patients with AMI and heart failure.
