The value of multi-parameter fMRI in evaluating renal blood flow and oxygenation levels in rats with cold ischemia-reperfusion injury

Abstract
Objective To explore the value of arterial spin labeling(ASL)and blood oxygen level dependent(BOLD)techniques in assessing microscopic changes in renal blood flow and blood oxygenation in rats with cold ischemia-reperfusion injury(CIRI)of the kidney.Methods A total of 100 healthy male SD rats were randomly divided into 4 groups:cold ischemia 1-hour group(CIRI 1 h),cold ischemia 2-hour group(CIRI 2 h),cold ischemia 4-hour group(CIRI 4 h),and sham operation group(CIRI 0 h),with 25 rats in each group.Five rats from each group were randomly selected at pre-operation and at 1 hour,1 day,2 days,and 5 days post-operation to undergo ASL and BOLD MRI.The renal cortical blood flow(RBFCo),renal cortical T2*values(T2*Co),outer stripe of outer medulla T2 values(T2*OSOM),and inner stripe of outer medulla T2 values(T2*ISOM)values were measured.The left kidney of the rats was then removed for pathological analysis and tubular injury scoring.One-way ANOVA was used to analyze the differences in MRI parameters and tubular injury scores among different groups at various time points and within groups at different time points.Pearson correlation analysis was conducted to determine the correlations between MRI parameters and between MRI parameters and tubular injury scores.Result At 1 hour post-operation,the RBFCo,T2*Co,and T2*OSOM values in the cold ischemia group were significantly lower than pre-operation values(all P<0.05).By 5 days post-operation,the RBFCo,T2*Co,T2*OSOM,and T2*ISOM values in the CIRI 0 h and CIRI 1 h groups had returned to the preoperative level(all P>0.05),whereas these values in the CIRI 4 h group remained lower than pre-operation levels(all P<0.05).At 1 hour,1 day,and 2 days post-operation,the T2*OSOM values in the CIRI 4 h group were lower than those in the other groups(all P<0.05).At 5 days post-operation,the RBFCo,T2*Co,T2*OSOM and T2*ISOM values in the CIRI 4 h group were lower than those in the CIRI 0 h group(all P<0.05).The tubular injury scores at all post-operation time points in the CIRI 2 h and CIRI 4 h groups were higher than their pre-operation scores(all P<0.05),with scores decreasing over time.The CIRI 4 h group had higher scores than the CIRI 0 h and CIRI 1 h groups at all post-operation time points.The CIRI 2 h group had higher scores than the CIRI 0 h and CIRI 1 h groups at 1 hour,2 days,and 5 days post-operation(all P<0.05).The RBFCo,T2*Co,T2*OSOM,and T2*ISOM values were moderately negatively correlated with the renal tubular injury scores(r=-0.714,-0.689,-0.518,-0.579,all P<0.001).The RBFCo values were moderately positively correlated with the T2*Co,T2*OSOM,and T2*ISOM values(r=0.704,0.525,0.612,all P<0.001).The T2*Co values were moderately positively correlated with the T2*OSOM and T2*ISOM values(r=0.685,0.572,P<0.001).The T2*OSOM values were moderately positively correlated with the T2*ISOM values(r=0.606,P<0.001).Conclusions ASL and BOLD imaging can dynamically assess microscopic changes in tissue blood flow and oxygenation level following renal CIRI,providing imaging evidence for early detection of renal CIRI.关键词
