血清L/A、NRG-1、Tie-2水平与首发精神分裂症患者临床症状严重程度的相关性
Correlation between Serum L/A,NRG-1,Tie-2 Levels and the Severity of Clinical Symptoms in Patients with First-episode Schizophrenia

Abstract
Objective To investigate the correlation of serum leptin(LEP)to adiponectin ratio(L/A),neuregulin-1(NRG-1),angiopoietin receptor tyrosine kinase 2(Tie-2)levels with the severity of clinical symptoms and their value in evaluation of cognitive dysfunction in patients with first-episode schizophrenia.Methods A total of 103 patients with first-episode schizophrenia admitted from May 2021 to May 2023 were selected as the research group,and 103 normal healthy volunteers undergoing physical examination during the same period were selected as the control group.Serum levels of L/A,NRG-1 and Tie-2 were compared between the two groups,and clinical symptoms[Positive and Negative Symptom Scale(PANSS),Brief Psychiatric Rating Scale(BPRS)scores],serum levels of L/A,NRG-1 and Tie-2 were compared between patients with cognitive dysfunction and normal cognitive function in the research group.The correlation between serum L/A,NRG-1,Tie-2 levels and the severity of clinical symptoms and the value of combined detection in the evaluation of cognitive dysfunction were analyzed.Results The levels of leptin and L/A in the research group were higher than those in the control group,while the levels of adiponectin,NRG-1 and Tie-2 in the research group were lower than those in the control group(P<0.01).In the research group,the scores and total scores of PANSS subscales,the scores and total scores of BPRS scales,and the levels of leptin and L/A in patients with cognitive dysfunction were higher than those in patients with normal cognitive function,while the levels of adiponectin,NRG-1 and Tie 2 levels were lower than those in patients with normal cog-nitive function(P<0.01).Serum L/A level was positively correlated with the score and total score of PANSS subscales and the score and total score of BPRS scale,while NRG-1 and Tie-2 levels were negatively correlated with the score and total score of PANSS subscale and total score of BPRS scale(P<0.01).The area under the receiver operating characteristic(ROC)curve(AUC)of serum L/A,NRG-1 and Tie-2 for evaluating cognitive dysfunction was 0.764,0.708 and 0.755,respective-ly,and the optimal cut-off values were 6.81,8.52 pg/mL and 1 962.62 pg/mL,respectively.The high expression of serum L/A,the low expression of NRG-1 and Tie-2 suggested that the risk of cognitive dysfunction increased by 5.237,6.172 and 4.538 times,respectively.The AUC of L/A combined with NRG-1,NRG-1 combined with Tie-2,L/A combined with Tie-2,and combination of the three was 0.882,0.868,0.876 and 0.932,respectively.Conclusion Serum L/A,NRG-1 and Tie-2 are significantly correlated with the severity of clinical symptoms in patients with first-episode schizophrenia,and abnor-mal expression increases the risk of cognitive dysfunction;therefore,the value of joint evaluation of cognitive dysfunction is more reliable.关键词
