目的 分析类风湿关节炎患者血清中长链非编码RNA GATA结合蛋白6 反义RNA1(lncRNA GATA6-AS1)和GATA结合蛋白6(GATA6)信使RNA(mRNA)水平与疾病活动度指标、免疫功能及疾病严重程度的关系.方法 选取2021年4 月至2023 年3 月我院收治的74 例类风湿关节炎缓解期患者作为缓解期组,81 例类风湿关节炎活动期患者作为活动期组,另选取同期体检健康者 80 例为对照组.收集受试者一般资料和疾病活动度指标[类风湿因子(RF)、血细胞沉降率(ESR)、C反应蛋白(CRP)、抗环瓜氨酸肽(CCP)抗体、肿瘤坏死因子(TNF)-α、28 处关节疾病活动度(DAS28)评分];荧光定量PCR法检测血清中lncRNA GATA6-AS1 和GATA6 mRNA水平;并检测受试者免疫功能指标[免疫球蛋白(Ig)A、IgM、IgG、CD3+T、CD4+T、CD8+T细胞,计算CD4+/CD8+比值].分析类风湿关节炎患者血清lncRNA GATA6-AS1、GATA6 mRNA、疾病活动度指标、免疫功能指标的相关性,影响类风湿关节炎患者活动期发生的危险因素,血清lncRNA GATA6-AS1、GATA6 mRNA对类风湿关节炎患者活动期发生的预测价值.结果 对照组、缓解期组、活动期组RF、ESR、CRP、抗CCP抗体、TNF-α、DAS28 评分、血清 lncRNA GATA6-AS1、GATA6 mRNA、IgA、IgM、IgG、外周血 CD3+T、CD4+T、CD8+T 细胞比例、CD4+/CD8+比值水平依次升高(P<0.05);风湿关节炎患者血清lncRNA GATA6-AS1 与GATA6 mRNA呈正相关(P<0.05);血清lncRNA GATA6-AS1 和GATA6 mRNA水平均与RF、ESR、CRP、抗CCP抗体、TNF-α、DAS28 评分、IgA、IgM、IgG、CD3+T细胞、CD4+T细胞、CD8+T细胞和CD4+/CD8+比值呈正相关(P<0.05);RF、ESR、CRP、抗 CCP 抗体、TNF-α、DAS28 评分、lncRNA GATA6-AS1、GATA6 mRNA均是影响类风湿关节炎患者活动期发生的独立危险因素(P<0.05);血清lncRNA GATA6-AS1、GATA6 mRNA、二者联合预测类风湿关节炎患者活动期发生的曲线下面积(AUC)分别为0.843、0.861、0.928;二者联合预测的AUC高于血清lncRNA GATA6-AS1、GATA6 mRNA水平各自单独预测的AUC(P<0.05).结论 类风湿关节炎患者血清中lncRNA GATA6-AS1 和GATA6 mRNA呈现高水平,且活动期患者二者水平更高,二者与疾病活动度指标、免疫功能以及疾病严重程度密切相关,可以很好地预测类风湿关节炎患者活动期的发生.

Objective To analyze the relationship between long non-coding RNA GATA binding protein 6 antisense RNA1(lncRNA GATA6-AS1)and GATA binding protein 6(GATA6)mRNA levels in serum of rheumatoid arthritis patients and disease ac-tivity indicators,immune function and disease severity.Methods A total of 74 patients in remission stage of rheumatoid arthritis in our hospital from April 2021 to March 2023 were selected as the remission stage group,81 patients in active stage of rheumatoid ar-thritis were selected as active stage group,and another 80 healthy subjects in the same period were selected as control group.General data and disease activity indicators[rheumatoid factor(RF),cell sedimentation rate(ESR),C-reactive protein(CRP),anti-cyclic citrullinated peptide(CCP)antibody,tumor necrosis factor(TNF)-α,28 joint disease activity(DAS28)score]were collected.The ln-cRNA GATA6-AS1 and GATA6 mRNA in serum were detected by fluorescence quantitative PCR.Immune function indexes of the subjects[immunoglobulin(Ig)A,IgM,IgG,CD3+T,CD4+T,CD8+T cells,CD4+/CD8+ratio]were detected.The correlation of serum lncRNA GATA6-AS1,GATA6 mRNA and disease activity indicators,immune function indicators in patients with rheumatoid arthritis,the risk factors affecting the occurrence of patients with rheumatoid arthritis in the active stage,predictive value of serum ln-cRNA GATA6-AS1 and GATA6 mRNA in patients with rheumatoid arthritis during active phase were all analyzed.Results RF,ESR,CRP,anti-CCP antibody,TNF-α,DAS28 score,serum lncRNA GATA6-AS1,GATA6 mRNA,IgA,IgM,IgG,the percent-age of CD3+T,CD4+T,CD8+T cells in peripheral blood and the ratio of CD4+/CD8+were increased in control group,remission stage group and active stage group(P<0.05);lncRNA GATA6-AS1 was positively correlated with GATA6 mRNA in patients with rheumatoid arthritis(P<0.05);serum lncRNA GATA6-AS1 and GATA6 mRNA levels were positively correlated with RF,ESR,CRP,anti-CCP antibody,TNF-α,DAS28 score,IgA,IgM,IgG,CD3+T cells,CD4+T cells,CD8+T cells and CD4+/CD8+ra-tio(P<0.05);RF,ESR,CRP,anti-CCP antibody,TNF-α,DAS28 score,lncRNA GATA6-AS1 and GATA6 mRNA were inde-pendent risk factors affecting the occurrence of patients with rheumatoid arthritis in active stage(P<0.05);Area under curve(AUC)of serum lncRNA GATA6-AS1,GATA6 mRNA and their combination predicted the occurrence of active phase in patients with rheu-matoid arthritis were 0.843,0.861 and 0.928,respectively;the AUC predicted by the combination was greater than that predicted by serum lncRNA GATA6-AS1 and GATA6 mRNA levels separately(P<0.05).Conclusion The serum levels of lncRNA GA-TA6-AS1 and GATA6 mRNA in patients with rheumatoid arthritis are high,and the levels of both are higher in patients with active stage,which are closely related to disease activity indicators,immune function and disease severity,and can well predict the occur-rence of patients with rheumatoid arthritis in active stage.
