目的 对TWIST2、SNAI1、TPX2 及Aurora-A表达水平与喉鳞癌患者预后的相关性进行分析.方法 将2016 年1 月至2020 年12 月于我院诊治的103 例喉鳞癌患者纳入研究,术前检测患者血TPX2 水平,术中留取肿瘤组织,检查肿瘤组织中TWIST2、SNAI1 及Aurora-A表达水平.根据患者2 年随访结果将患者分为预后良好组(n=71)及预后不良组(n=29),比较并分析两组患者一般资料及实验室指标,绘制并应用受试者工作(ROC)曲线评估影响因素预测患者预后的诊断价值.结果 预后良好组患者的年龄,肿瘤Ⅳ期、低分化-未分化及SNAI1、Aurora-A及TPX2 表达水平均显著低于预后不良组,而TWIST2 显著高于预后不良组(均P<0.05);CoX分析示,高SNAI1、Aurora-A及TPX2 表达水平是患者预后的独立危险因素,而高 TWIST2 表达水平为预后独立保护因素;根据治疗方案及术式对患者进行分层,分析TWIST2、SNAI1、TPX2 及Aurora-A水平对预后的影响差异有统计学意义(均P<0.05);随访1 年时,ROC曲线示TWIST2、SNAI1、Aurora-A及TPX2 表达水平联合预测患者预后的诊断效能与TWIST2、SNAI1、Aurora-A及TPX2 单一诊断效能相比较,差异无统计学意义(均P<0.05),TWIST2、SNAI1、Aurora-A及TPX2 表达水平联合预测患者预后的敏感度为100.0%,特异度为91.6%;随访2 年时,ROC曲线示TWIST2、SNAI1、Aurora-A及TPX2 表达水平联合预测患者预后的诊断效能显著高于TWIST2、SNAI1、Aurora-A及TPX2 单一诊断(均P<0.05),TWIST2、SNAI1、Aurora-A及TPX2 表达水平联合预测患者预后的敏感度为93.1%,特异度为 87.3%.结论 TWIST2、SNAI1、Aurora-A及TPX2 联合诊断能有效预测患者预后,具有较高特异度及敏感度.

Objective To analyze correlation between TWIST2,SNAI1,TPX2 and Aurora-A expression and prognosis of laryngeal squamous cell carcinoma patients.Methods From January 2016 to December 2020,103 patients with laryngeal squa-mous cell carcinoma diagnosed and treated in our hospital were included.Blood TPX2 were detected before surgery,and tumor tis-sues were collected during surgery to detect TWIST2,SNAI1 and Aurora-A expression in tumor tissues.According to 2-year fol-low-up results,patients were divided into good prognosis group(n=71)and poor prognosis group(n=29).General data and labo-ratory indicators of patients in two groups were compared and analyzed.Receiver operating(ROC)curve was drawn and used to e-valuate the diagnostic value of influencing factors in predicting patients'prognosis.Results The age,tumor stageⅣ,poorly differ-entiated to undifferentiated,SNAI1,Aurora-A and TPX2 expression of patients in good prognosis group were significantly lower than those in poor prognosis group,while TWIST2 was significantly higher than that in poor prognosis group(all P<0.05).CoX analy-sis showed that high SNAI1,Aurora-A and TPX2 was an independent risk factor for the prognosis of patients,while high TWIST2 expression was an independent protective factor for the prognosis.Patients were stratified according to treatment regimens and sur-gical methods,and effects of TWIST2,SNAI1,TPX2 and Aurora-A levels on the prognosis were also statistically significant(all P<0.05).After1-year follow-up,ROC curve showed that diagnostic efficacy of the combination of TWIST2,SNAI1,Aurora-A,and TPX2 expression levels in predicting the prognosis of patients was not statistically significant compared with that of TWIST2,SNAI1,Aurora-A,and TPX2 alone(all P<0.05),sensitivity of TWIST2,SNAI1,Aurora-A and TPX2 expression combined to pre-dict the prognosis of patients was 100.0%,and the specificity was 91.6%.During follow-up of 2 years,ROC curve showed that di-agnostic efficacy of the combination of TWIST2,SNAI1,Aurora-A,and TPX2 expression in predicting the prognosis of patients was significantly higher than that of TWIST2,SNAI1,Aurora-A,and TPX2 alone(all P<0.05),the sensitivity of TWIST2,SNAI1,Au-rora-A and TPX2 expression combined to predict the prognosis of patients was 93.1%,and the specificity was 87.3%.Conclusion TWIST2,SNAI1,Aurora-A and TPX2 combined diagnosis could effectively predict prognosis of patients with high specificity and sensitivity.
