甲状腺功能亢进症患者甲状腺激素水平变化及其与冠心病发生关系研究OACSTPCD
Changes of thyroid hormone levels in patients with hyperthyroidism and their relationship with occurrence of coronary heart disease
目的:观察甲状腺功能亢进症(甲亢)患者甲状腺激素水平变化,分析其与冠心病(CHD)发生的关系.方法:选取甲亢患者144例,根据是否合并CHD分为研究组(甲亢合并CHD,68例)与对照组(单纯甲亢,76例).比较两组患者一般资料及甲状腺激素水平.采用Logistic回归分析甲亢患者合并CHD的影响因素.采用受试者工作特征(ROC)曲线分析甲状腺激素对甲亢合并CHD的预测价值.采用Pearson相关性分析冠脉血管病变支数与甲状腺激素水平的关系.结果:研究组患者年龄、吸烟史、TG及促甲状腺激素(TSH)水平高于对照组,游离三碘甲状腺原氨酸(FT3)水平低于对照组(均P<0.05).年龄、吸烟史、甘油三酯(TG)、FT3及TSH水平为甲亢患者合并CHD的独立影响因素(均P<0.05).年龄、TG、FT3、TSH对甲亢合并CHD均有一定预测价值(均P<0.05),其中FT3预测甲亢合并CHD的敏感度较高,TSH预测的特异度较高.多支病变组患者TSH水平高于双支病变组与单支病变组,且双支病变组高于单支病变组(均P<0.05).冠脉血管病变支数与TSH水平呈正相关(r=0.469,P<0.05).结论:FT3、TSH水平对甲亢患者发生CHD具有较高的预测价值,TSH水平与甲亢合并CHD患者冠状动脉病变支数密切相关.
Objective:To observe the changes of thyroid hormone levels in patients with hyperthyroidism,and to analyze their relationship with occurrence of coronary heart disease(CHD).Methods:A total of 144 patients with hy-perthyroidism were selected and divided into study group(hyperthyroidism complicated with CHD,68 cases)and control group(simple hyperthyroidism,76 cases)according to whether they had CHD or not.The general data and thyroid hormone levels of the two groups were compared.Logistic regression was used to analyze the influencing fac-tors of CHD in patients with hyperthyroidism.ROC curve was used to analyze the predictive value of thyroid hor-mone for hyperthyroidism complicated with CHD.Pearson correlation analysis was used to analyze the relationship between the number of coronary artery lesions and thyroid hormone levels.Results:The age,smoking history,TG and TSH levels of the study group were higher than those of the control group,and FT3 level was lower than that of the control group(all P<0.05).Age,smoking history,TG,FT3 and TSH levels were independent influencing factors for CHD in patients with hyperthyroidism(all P<0.05).Age,TG,FT3 and TSH had certain predictive value for hyperthyroidism complicated with CHD(all P<0.05),and FT3 had a higher sensitivity in predicting hyperthyroidism combined with CHD,and TSH had a higher specificity.TSH level in multi-vessel disease group was higher than that in double-vessel disease group and single-vessel disease group,and TSH level in double-vessel disease group was higher than that in single-vessel dis-ease group(all P<0.05).The number of diseased coronary vessels was positively correlated with TSH level(r=0.469,P<0.05).Conclusion:FT3 and TSH levels have high predictive value for CHD in patients with hyperthyroidism,and TSH level is closely related to the number of diseased coronary vessels in patients with hyperthyroidism and CHD.
