心肺超声及斑点追踪成像技术对肺炎相关脓毒症患者预后的评估价值
The evaluation value of cardio-pulmonary ultrasound and spot tracking imaging technology in prognosis of patients with pneumonia related sepsis

Abstract
Objective To analyze the prognostic value of cardio and lung ultrasound parame-ters and spot tracking imaging technology in patients with pneumonia related sepsis.Methods Select 42 patients with sepsis caused by pneumonia who were initially admitted to the intensive care unit(ICU)or urgently consulted by ICU physicians.Monitor routine cardio and lung ultrasound parame-ters in the early stages of disease,and use the EchoPAC program to perform offline analysis of the echocardiography to obtain the global longitudinal strain(GLS)of the left ventricular long axis.Col-lect data of clinical monitoring and laboratory tests.According to the prognosis,there were 13 cases in the poor prognosis group and 29 cases in the good prognosis group.Evaluate the diagnostic value of each indicator using the receiver operating characteristic(ROC)curve.Results Clinical parameters:SOFA score,respiratory rate,proportion of mechanical ventilation,proportion of hemodynamic in-stability,lactic acid level in poor prognosis group were higher than those in good prognosis group,and mean arterial pressure level was lower than that in good prognosis group(P＜0.05).Ultrasonic pa-rameters:left ventricular outflow tract velocity-time integral(VTI)and maximum peak velocity(Vmax),cardiac output(CO),left ventricular ejection fraction(LVEF),GLS of left ventricular(GLS-Avg),three chamber,four chamber,two chamber GLS(GLS-A3C,GLS-A4C、GLS-A2C)in poor prognosis group were lower than those in good prognosis group,left ventricular end diastolic diameter(LVEDD),left ventricular filling pressure(E/e'),lung ultrasound score of the anterior chest wall was higher than that of the group with good prognosis(P＜0.05).The areas under the ROC curves of SOFA score,VTI,Vmax,CO,LVEDD,E/e',LVEF,GLS-Avg,LUS of the anteri-or chest wall and lactic acid were 0.897,0.797,0.789,0.788,0.725,0.699,0.794,0.707,0.688,and 0.736,respectively.Conclusions Early application of cardio-pulmonary ultrasound and spot tracking imaging parameters has good prognostic value for patients with pneumonia related sepsis.关键词
