N/LPR在预测急性呼吸窘迫综合征患者病情和临床结局中的意义
The significance of N/LPR in predicting the condition and clinical outcomes of patients with acute respir-atory distress syndrome

Abstract
Objective To investigate whether neutrophils to lymphocytes and platelets ratio(N/LPR)can be used to assess acute respiratory distress syndrome(ARDS)and predict the 28-day mortality risk.Methods The clinical data of 132 ARDS patients admitted to the Department of Re-spiratory and Critical Care Medicine in Our Hospital from January 2019 to July 2022 were retrospec-tively collected.According to Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ),all patients were divided into low-medium risk group(60 cases)(≤ 20 points)and high-risk group(72 cases)(>20 points).The survival status of ARDS patients within 28 days after diagnosis was followed up and recorded.All patients were divided into survival group(93 cases)and death group(39 cases).The difference of N/LPR in different groups was compared.Results The N/LPR of ARDS patients in high-risk group was significantly higher than that in low-middle-risk group(P<0.001).By Spearman correlation analysis,N/LPR in ARDS patients was positively correlated with APACHE Ⅱ score and Sequential Organ Failure Assessment Score(SOFA)(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of N/LPR for predicting ARDS was 0.822,and the specificity and sensitivity were 71.7%and 79.2%,respectively.The N/LPR of ARDS patients in the death group was significantly higher than that in the survival group(P<0.001).Logistic regres-sion analysis showed that APACHE Ⅱ score,NLR and N/LPR were independent predictors of 28-day mortality risk in ARDS patients(P<0.05).When the optimal cut-off value was 2.72,the AUC of N/LPR in predicting the 28-day death risk of ARDS patients could reach 0.864,which was higher than APACHE Ⅱ score,NLR,PLR(0.794,0.791,0.714).Kaplan-Meier analysis showed that pa-tients with high N/LPR(≥2.72)had significantly shorter overall survival(P<0.05).Conclusions Elevated N/LPR is associated with increased severity of ARDS and increased risk of death,and N/PLR holds promise as a biomarker for assessing ARDS severity and predicting 28-day risk of death.关键词
