New directions in myocardial protection:the comprehensive remote ischemic conditioning

Abstract
Remote ischemic conditioning(RIC)is the reversible ischemia-reperfusion of the vascular bed,tissue or organ to reduce the damage of the remote target tissue/organ after reperfusion.A recent large multicenter trial(CONDI-2/ERIC-PPCI)demonstrated no significant benefit for major clinical endpoints such as myocardial infarction size and cardiac death,and further studies are needed to a-dapt to current clinical settings.Comprehensive remote ischemic adaptation is a result of the combined application of remote ischemia in the treatment of myocardial infarction.Breakthroughs have been made in limiting the size of myocardial infarction and in preventing heart failure and ventricular remodeling after infarction.In this paper,the latest protective mechanisms,signal transduction pathways and carriers of RIC are described,and based on this,the additional myocardial protection benefits obtained by RIC under the condition that the same protective mechanisms are superimposed and different mechanisms are complementary at different treatment times are discussed,aiming to provide clinical ideas through the latest research progress of RIC.To explore individualized myocardial protection model suitable for patients.关键词
