硬膜外分娩镇痛联合经皮穴位电刺激在阴道分娩初产妇中的应用效果
Application effect of epidural labor analgesia combined with trans-cutaneous electrical acupoint stimulation in primiparas during vaginal labor

Abstract
Objective To investigate the application effect of epidural labor analgesia combined with transcutaneous electrical acupoint stimulation in primiparas during vaginal labor.Methods A total of 130 primiparas with vaginal labor admitted to Jinniu Maternity and Child Health Hospital of Chengdu from March 2022 to August 2023 were selected and divided into the control group and the observation group by the random number table method,with 65 cases in each group.The control group received epidural labor analgesia alone,and the observation group received epidural labor analgesia combined with transcutaneous electrical acupoint stimulation.The mean arterial pressure(MAP),heart rate(HR),pain score,labor course,neonatal Apgar score,overall incidence of adverse reactions,serum cortisol(Cor),and 5-hydroxytryptamine(5-HT)levels were compared between the two groups.Results Intra-group comparison:the MAP at the time of uterine opening of 3 cm(T1)in the observation group was compared with that before analgesia(T0),and the difference was statistically significant(P<0.01).HR and visual analogue scale(VAS)score between T1 and T0,at the time of uterine opening complete(T2)and T1 in the observation group were compared,and the differences were statistically significant(P<0.01).There were significant differences in MAP,HR,and VAS scores between T1 and T0,T2 and T1 in the control group(P<0.01).Comparison between groups:the MAP,HR,and VAS score of T2 and at the time of delivery(T3)in the observation group were lower than those in the control group(P<0.01).There were no significant differences in the course of labor,Apgar score at 1 min and 5 min,and the total incidence of adverse reactions between the two groups(P>0.05).Compared with T0,serum Cor level in both groups decreased at 30 min after delivery(T4),and 5-HT level increased at 3 days postpartum(T5)(P<0.01).Serum Cor level at T4 of the observation group was lower than that in the control group,and 5-HT level at T5 was higher than that in the control group(P<0.01).Conclusion Epidural labor analgesia combined with transcutaneous electrical acupoint stimulation in primiparas during vaginal labor has a good effect,can relieve labor pain,reduce serum Cor level,increase 5-HT level,and has safety.关键词
