目的 探讨急性心肌梗死(AMI)患者微小RNA-325-3p(miR-325-3p)水平与病情严重程度及预后的关系.方法 选取北京市怀柔区中医医院 2018 年 7 月至 2020 年 6 月收治的90 例AMI患者.采用实时荧光定量PCR(RT-qPCR)法检测受试者血浆miR-325-3p表达水平.依据Gensini评分将AMI组患者分为轻度病变组 28 例、中度病变组 36 例、重度病变组 26 例.随访 2 年,根据不同预后情况分为预后不良组 48 例、预后良好组 42 例.采用Spearman法分析AMI患者血浆miR-325-3p表达水平与Gensini评分、肌酸激酶同工酶(CK-MB)、脑利钠肽(BNP)、心肌肌钙蛋白I(cTnI)水平的相关性;采用Kaplan-Meier生存曲线分析miR-325-3p表达水平与AMI患者预后的关系;采用Cox回归分析AMI患者预后不良的影响因素.结果 轻度、中度、重度病变组三组患者miR5-32-3p表达水平及Gensini评分比较差异有统计学意义(F=39.031、78.573,P<0.01).与轻度、中度病变组相比,重度病变组患者血浆miR-325-3p表达水平降低;与轻度病变组相比,中度病变组患者血浆miR-325-3p表达水平降低(P<0.05).与预后良好组相比,预后不良组miR-325-3p表达水平降低(t=7.525,P<0.05).Spearman相关性分析显示,AMI患者血浆miR-325-3p表达水平与Gensini评分、CK-MB、BNP、cTnI水平均呈负相关(r=-0.616、-0.608、-0.574、-0.642,P<0.01).Kaplan-Meier法分析显示,miR-325-3p低表达组不良预后发生率高于miR-325-3p高表达组(χ2=18.581,P<0.01).多因素Cox回归分析表明,Killip分级(≥3级)、cTnI是影响AMI患者不良预后的危险因素,而miR-325-3p及LVEF是保护因素(HR=1.405、1.628、0.794、0.816,P<0.05).结论 AMI患者miR-325-3p表达水平与病情严重程度及预后关系密切,有望成为预测AMI患者不良预后的生物标志物.

Objective To investigate the relationship between microRNA-325-3p(miR-325-3p)level and the severity as well as prognosis in patients with acute myocardial infarction(AMI).Methods From July 2018 to June 2020,90 AMI patients admitted to Huairou District Traditional Chinese Medicine Hospital of Beijing were selected.The expression level of miR-325-3p in patients'plasma was detected by real-time quantitative PCR(RT-qPCR).According to Gensini score,AMI patients were classified into the mild disease group(28 cases),moderate disease group(36 cases),and severe disease group(26 cases).After 2 years of follow-up,they were grouped into the poor prognosis group(48 cases)and good prognosis group(42 cases)based on different prognosis.Spearman method was applied to analyze the correlation between plasma miR-325-3p expression and Gensini score,creatine kinase isoenzymes(CK-MB),B-type natriureticpeptide(BNP),as well as cardiactroponin(cTnI)in AMI patients.Kaplan-Meier survival curve was applied to analyze the relationship between miR-325-3p expression and the prognosis of AMI patients.Influencing factors for the poor prognosis in AMI patients were analyzed by Cox regression.Results The expression levels of miR-325-3p and Gensini scores among mild,moderate and severe disease groups were statistically significant different(F=39.031,78.573;P<0.01).Compared with the mild and moderate disease groups,plasma miR-325-3p expression level in the severe disease group was significantly decreased;compared with the mild disease group,plasma miR-325-3p expression level in the moderate disease group was significantly decreased(P<0.05).Compared with the good prognosis group,the expression level of miR-325-3p in the poor prognosis group was significantly decreased(t=7.525,P<0.05).Spearman correlation analysis showed that plasma miR-325-3p expression level in AMI patients was negatively correlated with Gensini score,CK-MB,BNP and cTnI(r=-0.616,-0.608,-0.574,-0.642;P<0.01).Kaplan-Meier analysis showed that the incidence of poor prognosis in the miR-325-3p low expression group was obviously higher than that in the miR-325-3p high expression group(χ2=18.581,P<0.01).Multivariate Cox regression analysis showed that Killip grade(≥grade 3)and cTnI were risk factors for the poor prognosis of AMI patients,while miR-325-3p and LVEF were protective factors(HR=1.405,1.628,0.794,0.816;P<0.05).Conclusion The expression level of miR-325-3p is closely related to the severity and prognosis in AMI patients,thus can be a potential biomarker for predicting the poor prognosis of AMI patients.
