Abstract
Objective To investigate the applicability of shear wave elastography(SWE)in evaluating Young's modulus changes of placenta in patients suffering from hypertensive disorder of pregnancy(HDP)and concomitant fetal intracranial hemorrhage(FICH),aiming to assist in early clinical diagnosis and intervention.Methods A total of 134 pregnant women diagnosed with HDP and grade Ⅱ placenta were included as the case group.They were further divided into Group A(28 cases with FICH)and Group B(106 cases without FICH)based on the presence of FICH.Additionally,129 pregnant women with normal pregnancy and grade Ⅱ placenta were selected as a control group.All subjects were assessed using SWE to measure the Young's modulus in the central and marginal areas of the placenta.The differences among these values were analyzed,and receiver operating characteristic(ROC)curves were constructed to evaluate the predictive efficiency of the Young's modulus in diagnosing HDP and FICH.Results The Young's modulus values of the placenta in the central and marginal areas in the case group were higher than those in the control group(P＜0.05).Within the case group,the Young's modulus values in both central and marginal areas in Group A were higher than those in Group B(P＜0.05).For Group A,a trend of increasing Young's modulus values in both central and marginal areas of placenta was observed correlating with the severity of FICH,though without statistical significance among different severity levels(P＞0.05).The ROC curve showed that Young's modulus values in the central and marginal areas of placenta in the case group predicted HDP with an AUC of 0.900 and 0.904,respectively,and in Group A,the predicted values for HDP complicated with FICH were 0.912 and 0.908,respectively.Conclusion SWE measurement of the placenta's Young's modulus offers significant predictive val-ue for HDP and associated FICH.It stands as a vital index for assessing the placental function in HDP patients,thereby ai-ding in clinical diagnosis and therapeutic strategies.Routine assessment of the Young's modulus is recommended for HDP patients to prevent fetal hypoxia,intracranial hemorrhage and other complications.关键词
