目的:对比分析正常对照组与干燥综合征(Sjogren's syndrome,SS)患者腮腺、颌下腺及舌下腺二维超声评分以及实时二维剪切波超声弹性成像(Two-dimensional shear wave elastography,2D-SWE)杨氏模量,评价两种超声检查技术对SS患者的诊断效能.方法:纳入 100 例非SS患者(正常对照组)和 90 例SS患者(SS组),对比两组研究对象腮腺、颌下腺、舌下腺的二维超声评分及 2D-SWE的杨氏模量,绘制受试者工作特征(Receiver operating characteristic,ROC)曲线,评估二维超声评分及 2D-SWE诊断SS的效能.结果:对照组和SS组 3 种腺体之间二维超声评分、杨氏模量的差异均具有统计学意义(P<0.01).腮腺二维超声评分曲线下面积(Area under the curve,AUC)高于颌下腺(P<0.01),颌下腺二维超声评分的AUC值高于舌下腺(P<0.01).根据约登指数,取最佳截断值时,腮腺二维超声评分诊断SS的灵敏度、特异度分别为 83.3%、93.0%;颌下腺二维超声评分诊断SS的灵敏度、特异度分别为 90.0%、51.0%;舌下腺二维超声评分诊断SS的灵敏度、特异度分别为 66.7%、56.0%.且腮腺、颌下腺以及舌下腺 2D-SWE诊断SS的效能均高于二维超声评分.结论:腮腺、颌下腺、舌下腺二维超声表现在诊断SS中具有一定的效能,且 2D-SWE对SS的诊断效能优于二维超声评分.

Objective:To assess the differences in two-dimensional ultrasonography and shear wave elastography(2D-SWE)of the parotid,submandibular and sublingual glands between the control and Sjogren's syndrome(SS)group,and to evaluate the clinical diagnostic efficiency of conventional ultrasonography and elastography in SS.Methods:This study enrolled 100 non-SS patients(control group)and 90 SS patients(SS group).The two-dimensional scores of ultrasonography and the elastic modulus of 2D-SWE in the parotid,submandibular and sublingual glands were analyzed.The receiver operating characteristic(ROC)curve was used to evaluate the diagnostic performance of the two-dimensional score and the speed of SWE in SS.Results:There were statistically significant differences in the two-dimensional scores and the elastic modulus of SWE of the three glands between the control group and the SS group(P<0.01).The area under the curve(AUC)value of parotid gland was higher than that of submandibular gland(P<0.01),and the AUC value of submandibular gland was higher than that of sublingual gland(P<0.01).Based on the Youden index,the sensitivity and specificity of parotid,submandibular and sublingual glands for diagnosing SS at the optimal cut-off value were 83.3%,93.0%,and 90.0%,51.0%,66.7%,and 56.0%,respectively.The AUC values of elastic modulus of SWE in parotid gland,submandibular gland and sublingual gland for diagnosing SS were higher than those of the two-dimensional score.Conclusions:Two-dimensional ultrasound of parotid gland,submandibular gland and sublingual gland has certain clinical diagnostic efficacy in the diagnosis of SS,and the diagnostic performance of elastic modulus of SWE in salivary gland is superior to that of two-dimensional score in SS.
