On Risk Factors for Depression after Ischemic Stroke

摘要

Abstract

Objective To explore the potential influencing factors of post-stroke depression(PSD)in patients with ischemic stroke and to provide a scientific basis for early identification and prevention of depression.Method By analyzing the detailed clinical data of 89 patients with ischemic stroke,the patients were divided into depression group(HAMD≥7 points)and non-depression group(HAMD<7 points)according to HAMD scale score.Using single-factor and multi-factor logistic regression analysis,supplemented by decision tree technology based on Chi-squared automatic interaction detector(CHAID),the main influencing factors of post-ischemic stroke depression were assessed.Results Single-factor analysis showed that the difference of the history and course of coronary heart disease,course of cerebral infarction,and plasma levels of IL-1,IL-6 and TNF-α between the two groups was statistically significant(P<0.05);Multiple Logistic regression analysis further determined that the history of coronary heart disease,IL-1 and IL-6 are independent risk factors for PSD(P<0.05),and TNF-α is an independent protective factor for PSD(P<0.05);The decision tree model showed plasma IL-6,IL-1 and long-term coronary heart disease course are influencing factors of depression in patients with ischemic stroke.Conclusion Patients with ischemic stroke,a history or long-term coronary heart disease,high serum IL-1 levels and high serum IL-6 levels are risk factors for PSD.
