CT灌注成像参数对急性脑梗死患者神经功能预后的预测价值
Predictive value of CT perfusion imaging on the neurological outcome after acute cerebral infarction

Abstract
Objective To analyze the predictive value of analytical CT perfusion imaging on the neurological outcome after acute cerebral infarction.Methods From January 2020 to January 2023,88 patients with acute cerebral infarction admitted to the Ninth People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine were selected as the study objects.The patients with modified Rankin scale(mRS)score≤2 were divided into good prognosis group(40 cases),and those with mRS score>2 were divided into poor prognosis group(48 cases).The differences between the general data of gender,age,NIHSS score,history of underlying diseases and the parameters related to CT perfusion imaging examination including CBV,CBF,TTP and MTT were compared.Results The differences of gender,history of underlying disease,and healthy CT perfusion imaging parameters were not significantly different between the two groups(P>0.05).The age of patients((54.35±8.74)years old),NIHSS score((9.23±4.75)points),TTP((18.27±3.62)s),MTT((6.85±2.43)s)were all lower than those of the poor prognosis group((60.64±9.90)years old,(12.08±4.79)points,(20.68±4.03)s,(8.21±3.22)s,respectively).The CBV((4.16±1.25)mL/100 g),CBF((60.24±12.57)mL/(100 g·min))were higher than those of the poor prognosis group((3.39±1.14)mL/100 g,(38.86±9.43)mL/(100 g·min),respectively).The differences were statistically significant(P<0.05).Logistic regression analysis showed that the parameters related to CT perfusion imaging examination,such as CBV,CBF,TTP,and MTT,were all independent risk factors affecting the neurological outcome after acute cerebral infarction,with statistically significant differences(P<0.05).The area under the ROC curve AUC,including CBV,CBF,TTP,and MTT,predicting neurological outcome after acute cerebral infarction was 0.8818,0.9217,0.9043,0.8775,respectively.Conclusion CT perfusion imaging can predict the prognosis of neurological function after acute cerebral infarction,with significant clinical value.关键词
