目的 探讨细胞角蛋白 19 片段(CYFRA21-1)、微小核糖核酸 21(miRNA-21)、中期因子(MK)与非小细胞肺癌病理特征的关系及其诊断价值.方法 选取南乐中兴医院 2022 年 2 月至 2023 年 2 月收治的 80 例非小细胞肺癌患者作为观察组,另选取 80 例健康志愿者作为对照组.比较观察组与对照组CYFRA21-1、miRNA-21、MK水平,分析CYFRA21-1、miRNA-21、MK水平与非小细胞肺癌病理特征的关系,分析CYFRA21-1、miRNA-21、MK单独及三者联合检测诊断非小细胞肺癌的价值并绘制受试者工作特征(ROC)曲线.结果 观察组CYFRA21-1、miRNA-21、MK水平高于对照组,差异均有统计学意义(P<0.05);鳞状细胞癌患者CYFRA21-1、miRNA-21、MK水平高于腺癌患者,差异均有统计学意义(P<0.05),美国联合肿瘤委员会TNM分期Ⅲ+Ⅳ期患者CYFRA21-1、miRNA-21、MK水平高于Ⅰ+Ⅱ期患者,差异均有统计学意义(P<0.05),有淋巴结转移患者CYFRA21-1、miRNA-21、MK水平高于无淋巴结转移患者,差异均有统计学意义(P<0.05);CYFRA21-1、miRNA-21、MK三者联合检测诊断非小细胞肺癌的曲线下面积(AUC)值高于单项检测,具有统计学意义(P<0.05).结论 CYFRA21-1、miRNA-21、MK水平与非小细胞肺癌病理类型、肿瘤分期及淋巴结转移有关,联合上述指标可提高非小细胞肺癌的诊断效能.

Objective To explore the relationship and diagnostic value of cytokeratin 19 fragment(CYFRA21-1),microRNA-21(miRNA-21),and midkine(MK)with the pathological characteristics of non-small cell lung cancer(NSCLC).Methods Eighty NSCLC patients treated in Nanle Zhongxing Hospital from February 2022 to February 2023 were selected as the observation group,and another 80 healthy volunteers were selected as the control group.The levels of CYFRA21-1,miRNA-21,and MK in the observation group and the control group were compared,the relationship between the levels of CYFRA21-1,miRNA-21,and MK and the pathological characteristics of NSCLC was analyzed,and the value of CYFRA21-1,miRNA-21,and MK alone and in combination in diagnosing NSCLC was analyzed and the receiver operating characteristic(ROC)curve was drawn.Results The levels of CYFRA21-1,miRNA-21,and MK in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05);the levels of CYFRA21-1,miRNA-21,and MK in patients with squamous cell carcinoma were higher than those in patients with adenocarcinoma,and the differences were statistically significant(P<0.05);the levels of CYFRA21-1,miRNA-21,and MK in patients with stage Ⅲ+Ⅳ according to the American Joint Committee on Cancer TNM staging were higher than those in patients with stage Ⅰ+Ⅱ,and the differences were statistically significant(P<0.05);the levels of CYFRA21-1,miRNA-21,and MK in patients with lymph node metastasis were higher than those in patients without lymph node metastasis,and the differences were statistically significant(P<0.05);the area under the curve(AUC)value of CYFRA21-1,miRNA-21,and MK in combination in diagnosing NSCLC was higher than that of single detection,and the difference was statistically significant(P<0.05).Conclusion The levels of CYFRA21-1,miRNA-21,and MK are related to the pathological type,tumor stage,and lymph node metastasis of NSCLC,and the combination of these indicators can improve the diagnostic efficiency of NSCLC.
