超声引导下颈脊神经后支射频热凝治疗颈型颈椎病的效果OA
The Effect of Improvement Effect of Ultrasound-guided Radiofrequency Thermocoagulation in the Posterior Branch of Cervical Spinal Nerve on the Treatment of Cervical Spondylosis
目的:分析超声引导下颈脊神经后支射频热凝治疗颈型颈椎病的效果,探讨该治疗方法对患者疼痛症状改善效果.方法:选取福建省龙岩市第二医院康复科 2021 年 1 月—2023 年 2 月收治的88 例颈型颈椎病患者作为研究对象,按随机数字表法将其分为参照组与研究组,每组 44 例,参照组实施常规治疗(针刺联合镇痛消炎药物),研究组在参照组常规治疗基础上采用超声引导下颈脊神经后支射频热凝治疗.比较两组治疗前后的疼痛评分、睡眠质量、颈椎功能障碍指数(NDI)评分、颈椎活动度.结果:治疗后 1、3、6 个月,研究组疼痛评分均低于参照组,差异均有统计学意义(P<0.05).治疗后 1、3、6 个月,研究组失眠评分均低于参照组,差异均有统计学意义(P<0.05).治疗后 1、3、6 个月,研究组NDI评分均低于参照组,差异均有统计学意义(P<0.05);治疗后 6 个月,研究组前屈、后伸、左旋、右旋活动度均大于参照组,差异均有统计学意义(P<0.05).结论:超声引导下颈脊神经后支射频热凝治疗可显著减轻颈型颈椎病患者疼痛,改善颈椎功能,提高颈椎活动度和睡眠质量.
Objective:To analyze the effectiveness of ultrasound-guided radiofrequency thermocoagulation in the posterior branch of the cervical spine nerve on the treatment of cervical spondylosis,and to explore the effect of this treatment method on improving pain symptoms in patients.Method:A total of 88 patients with cervical spondylosis admitted to the Rehabilitation Department of the Second Hospital of Longyan,Fujian Province from January 2021 to February 2023 were selected as the study subjects,they were divided into a reference group and a study group by random number table method,with 44 cases in each group.The reference group received routine treatment(acupuncture combined with analgesic and anti-inflammatory drugs),while the study group used ultrasound-guided radiofrequency thermocoagulation in the posterior branch of the cervical spinal nerve on the basis of conventional treatment in the reference group.The pain scores,sleep quality,neck disability index(NDI)scores,and cervical range of motion before and after treatment between two groups were compared.Result:At 1,3,and 6 months after treatment,the pain scores in the study group were lower than those in the reference group,the differences were statistically significant(P<0.05).At 1,3,and 6 months after treatment,the insomnia scores of the study group were lower than those in the reference group,the differences were statistically significant(P<0.05).At 1,3,and 6 months after treatment,the NDI scores of the study group were lower than those of the reference group,the differences were statistically significant(P<0.05).After 6 months of treatment,the range of motion of the study group in flexion,extension,left rotation,and right rotation were higher than those of the reference group,the differences were statistically significant(P<0.05).Conclusion:Ultrasound-guided radiofrequency thermocoagulation in the posterior branch of the cervical spinal nerve significantly reduces pain,improves cervical function,improves cervical mobility,and improves sleep quality in patients with cervical spondylosis.
