Abstract
Objective:To explore the value of Hydromorphone combined with Bupivacaine on patient-controlled epidural analgesia after cesarean section.Method:A total of 92 gravidas who planned to undergo cesarean section in Maternity&Child Care Center of Xinyu from January 2020 to June 2023 were selected as the study objects,and were divided into observation group(n=46)and control group(n=46)according to random number table method.Both groups received patient-controlled epidural analgesia after surgery.The control group used Bupivacaine,and the observation group used Hydromorphone+Bupivacaine.Postoperative pain,analgesia pump use,lactation,adverse reactions and recovery quality were compared between the two groups.Result:The VAS scores of the observation group were lower than those of the control group at 4,8,12,24 and 48 h after surgery,the first compression time of analgesia pump was later than that of control group,the number of analgesia pump compressions within 48 h after surgery was less than that of control group,the start time of postpartum lactation was earlier than that of control group,each item scores and total scores of 40-item quality of recovery score(QoR-40)48 h after surgery were higher than those of control group,the differences were statistically significant(P<0.05).There were no significant differences in the incidence of adverse reactions and the proportion of adequate lactation volume within 48 hours after delivery between the two groups(P>0.05).Conclusion:Hydromorphone combined with Bupivacaine on patient-controlled epidural analgesia after cesarean section can effectively relieve postoperative pain,start postpartum lactation as soon as possible,ensure good recovery quality,and do not significantly increase adverse reactions.关键词
