泪河高度在儿童泪道阻塞诊疗中的应用研究

Abstract
Objective To quantitatively analyze the accuracy of tear meniscus height(TMH)in the diagnosis of lacrimal passage obstruction in children and evaluate its clinical value.Methods The clinical data of 18 children(18 eyes)with lacrimal passage obstruction in Beijing Tongen Hospital from August 2022 to March 2024 were analyzed retrospectively.The results of TMH and lacrimal passage irrigation before and after operation were collected.The accuracy of TMH and binocular TMH ratio in the diagnosis of lacrimal passage obstruction was evaluated.Results 18 children were included in this study.15 cases of preoperative TMH,10 postoperative TMH were collected,of which 8 cases had both preoperative and postoperative TMH.The preoperative TMH of affected eyes was significantly higher than that of healthy eyes and postoperative TMH,which were 0.56±0.45mm,0.22±0.14mm and 0.23±0.06mm,respectively.The TMH ratio of affected side to contralateral side(2.48±1.36)was significantly higher than that after operation(1.33±0.42)(P<0.05).Spearman correlation analysis showed a significant correlation between TMH and the results of lacrimal passage irrigation.The higher the degree of lacrimal passage obstruction,the higher the TMH.The results of binary logistics regression analysis showed that both TMH and binocular TMH ratio could well indicate lacrimal passage obstruction.ROC curve confirmed that TMH and binocular TMH ratio were effective in predicting lacrimal passage obstruction,and the cutoff values were 0.28mm and 1.72,respectively.Conclusion TMH and binocular TMH ratio have good sensitivity and specific-ity in the evaluation of lacrimal passage obstruction in children,especially for children who are younger and cannot cooper-ate with lacrimal passage irrigation,they have the advantages of non-invasive,rapid and good matching.Binocular TMH ratio greater than 1.72 can be used as an effective index for the diagnosis of monocular lacrimal passage obstruction.关键词
