目的 探讨血清心肌肌球蛋白结合蛋白C(cMyBP-C)对急性心肌梗死(AMI)早期的诊断价值.方法 选取2022年1月至2023年1月该院收治的90例AMI患者作为观察组.另选取该院同期收治的90例非AMI患者作为对照组.检测并比较两组cMyBP-C、血清肌酸激酶同工酶(CK-MB)、肌红蛋白(Myo)、超敏心肌肌钙蛋白T(hscTnT)水平,分析AMI患者血清cMyBP-C水平与CK-MB、Myo、hscTnT水平的相关性.绘制受试者工作特征(ROC)曲线分析血清cMyBP-C、CK-MB、Myo、hscTnT单独及cMyBP-C与hscTnT联合检测对AMI早期的诊断价值.采用Spearman相关分析AMI患者血清cMyBP-C水平与CK-MB、Myo、hscT-nT水平的相关性.结果 观察组血清cMyBP-C、CK-MB、Myo、hscTnT水平均高于对照组(P<0.05)o ROC曲线分析结果显示,血清cMyBP-C、CK-MB、Myo、hscTnT单独及cMyBP-C与hscTnT联合检测诊断AMI的曲线下面积(AUC)分别为 0.96、0.76、0.81、0.95、0.98,血清 cMyBP-C、CK-MB、Myo、hscTnT 单独检测的AUC均低于cMyBP-C与hscTnT联合检测的AUC(P<0.05).Spearman相关分析结果显示,AMI患者cMyBP-C水平与CK-MB、Myo水平均呈正相关(r=0.381、0.409,P<0.05),与hscTnT水平无相关性(r=0.062,P>0.05).结论 血清cMyBP-C、CK-MB、Myo、hscTnT 4项指标均可作为AMI发生早期的诊断指标,cMyBP-C与hscTnT联合检测诊断AMI的价值优于上述4项指标单独检测.cMyBP-C在AMI早期诊断中具有重要价值,可作为临床诊断早期AMI的重要指标之一.

Objective To investigate the value of serum cardiac myosin binding protein C(cMyBP-C)in the early diagnosis of acute myocardial infarction(AMI).Methods A total of 90 AMI patients admitted to the hospital from January 2022 to January 2023 were selected as the observation group.In addition,90 non-AMI patients admitted to the hospital during the same period were selected as the control group.The levels of cMyBP-C,serum creatine kinase isoenzyme(CK-MB),myoglobin(Myo)and high-sensitivity cardiac troponin T(hscTnT)were detected and compared between the two groups,and the correlation between serum cMyBP-C level and CK-MB,Myo and hscTnT levels in AMI patients was analyzed.Receiver operating characteristic(ROC)curve was drawn to analyze the value of serum cMyBP-C,CK-MB,Myo,hscTnT alone and cMyBP-C combined with hscTnT in the early diagnosis of AMI.Spearman correlation analysis was used to analyze the correlation between serum cMyBP-C level and CK-MB,Myo,hscTnT levels in AMI patients.Results The levels of serum cMyBP-C,CK-MB,Myo and hscTnT in the observation group were higher than those in the control group(P<0.05).ROC curve analysis results showed that the area under the curve(AUC)of cMyBP-C,CK-MB,Myo,hscTnT alone and cMyBP-C combined with hscTnT in the diagnosis of AMI was 0.96,0.76,0.81,0.95,0.98 respectively.The AUC of serum cMyBP-C,CK-MB,Myo,and hscTnT alone was lower than that of the combined detection of cMyBP-C and hscTnT(P<0.05).Spearman correlation analysis showed that the level of cMyBP-C was positively correlated with the levels of CK-MB and Myo in AMI patients(r=0.381,0.409,P<0.05).There was no correlation between cMyBP-C level and hscTnT level(r=0.062,P>0.05).Conclusion Serum cMyBP-C,CK-MB,Myo and hscTnT can be used as early diagnostic indicators for AMI,and the combined detection of cMyBP-C and hscTnT in the diagnosis of AMI is better than the single detection of the above four indicators.cMyBP-C has an important value in the early diagnosis of AMI,and it can be used as one of the important indicators for the clinical diagnosis of early AMI.
