血清miR-503、miR-520h对妊娠期糖尿病患者并发子痫前期的诊断价值
Diagnostic value of serum miR-503 and miR-520h in patients with gestational diabetes mellitus complicated with preeclampsia

Abstract
Objective To investigate the diagnostic value of serum microrNA-503(miR-503)and micror-NA-520h(miR-520h)in patients with gestational diabetes mellitus(GDM)complicated with preeclampsia(PE).Methods A total of 102 GDM patients who underwent regular prenatal examination and established files in the Department of Obstetrics of Tangshan Central Hospital from December 2019 to December 2021 were selected as the research objects.According to the type of disease,they were divided into GDM+PE group(53 cases)and GDM group(49 cases).In addition,57 healthy pregnant women who underwent regular prena-tal examination and delivered in Tangshan Central Hospital during the same period were selected as the nor-mal pregnancy group.The related indicators of glucose and lipid metabolism[triglyceride(TG),total choles-terol(TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),free fatty acid(FFA),fasting insulin(FINS),fasting blood glucose(FBG),2 h postprandial blood glucose(2 h PBG)[levels and serum miR-503,miR-520h,serum cystatin C(CysC)and homocysteine(Hcy)levels were compared among the three groups.Adverse pregnancy outcomes were compared among the three groups.Mul-tivariate Logistic regression was used to analyze the risk factors of PE in GDM patients.The receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic value of miR-503,miR-520h alone and com-bined detection of the two indicators in GDM complicated with PE.Results There were no significant differ-ences in gestational age,age,pre-pregnancy body mass index and LDL-C level among the three groups(P＞0.05).The levels of TC,TG and FFA in the GDM+PE group were significantly higher than those in the GDM group and the normal pregnancy group,and the level of HDL-C was significantly lower than that in the GDM group and the normal pregnancy group,and the differences were statistically significant(P＜0.05).The levels of TC and FFA in GDM group were significantly higher than those in normal pregnancy group,and the differences were statistically significant(P＜0.05).The levels of FINS,FBG,2 h PG and HbA1c in GDM+PE group and GDM group were significantly higher than those in normal pregnancy group,and the differences were statistically significant(P＜0.05).The expression levels of serum miR-503,miR-520h,CysC and Hcy in the GDM+PE group were significantly higher than those in the GDM group and the normal pregnancy group(P＜0.05).The expression levels of serum miR-503,miR-520h,CysC and Hcy in GDM group were signifi-cantly higher than those in normal pregnancy group(P＜0.05).The total incidence of adverse pregnancy out-comes in GDM+PE group was 62.26％(33/53),in GDM group was 40.82％(20/49),and in normal preg-nancy group was 10.53％(6/57).The total incidence of adverse pregnancy outcomes in GDM+PE group was significantly higher than that in GDM group and normal pregnancy group,and the differences were statistically significant(X2=4.692,32.125,P＜0.05).The total incidence of adverse pregnancy outcomes in GDM group was significantly higher than that in normal pregnancy group,and the difference was statistically significant(X2=13.059,P＜0.05).There were statistically significant differences in the incidence of postpartum hemor-rhage,premature or post-term birth,placental abruption,neonatal asphyxia,and intrauterine growth restric-tion among the three groups(P＜0.05).Multivariate Logistic regression analysis showed that increased CysC level,increased Hcy level,increased miR-503 expression level and increased miR-520h expression level were risk factors for GDM complicated with PE(P＜0.05).ROC curve analysis results showed that the area under the curve(AUC)of serum miR-503,miR-520h alone and the combination of the two indicators in the diagno-sis of GDM complicated with PE were 0.755,0.847,0.935 respectively.The AUC of the combined detection of the two indicators was greater than that of miR-503 or miR-520h alone(Z 2item combined with miR-503=4.210,Z2item combined with miR-520h=2.708,P＜0.001,P=0.007).Conclusion The expression levels of serum miR-503 and miR-520h in GDM patients with PE are increased,which are positively correlated with adverse pregnancy out-comes.The combined detection of Mir-503 and Mir-520h is of great significance for the diagnosis of GDM complicated with PE.关键词
