骨水泥成形术联合鲑鱼降钙素治疗老年骨质疏松性胸腰椎骨折的效果观察OA
Curative Effect of Bone Cement Plasty Combined with Salmon Calcitonin in the Treatment of Senile Osteoporotic Thoracolumbar Fracture
目的:观察骨水泥成形术联合鲑鱼降钙素治疗老年骨质疏松性胸腰椎骨折的效果.方法:选取 2019年 1月—2022 年 10月我院收治的 60例老年骨质疏松性胸腰椎骨折患者为研究对象,按照顺序编码法分为试验组和参照组,每组 30例,2组均接受骨水泥成形术治疗,试验组在此基础上联合应用鲑鱼降钙素治疗,对比 2组Oswestry功能障碍指数(ODI)、疼痛视觉模拟(VAS)评分、骨折愈合、下床活动、疼痛缓解时间、临床疗效,观察伤椎恢复、骨密度改善情况.结果:术后3、6个月,试验组ODI评分均比参照组低,组间差异有统计学意义(P<0.05);术后1周、1个月,试验组VAS评分均比参照组低,组间差异有统计学意义(P<0.05);试验组骨折愈合、下床活动、疼痛缓解时间均比参照组短,组间差异有统计学意义(P<0.05);治疗前,2组后凸Cobb角、伤椎楔变角、伤椎前缘高度、右股骨颈骨密度、腰椎骨密度比较,组间差异无统计学意义(P>0.05);治疗后,2组后凸Cobb角、伤椎楔变角均较治疗前缩小,伤椎前缘高度、右股骨颈骨密度、腰椎骨密度均较治疗前升高,且试验组后凸Cobb角、伤椎楔变角均比参照组小,伤椎前缘高度、右股骨颈骨密度、腰椎骨密度均比参照组高,差异有统计学意义(P<0.05);试验组治疗总有效率为 96.67%,高于参照组的 80.00%,差异有统计学意义(P<0.05).结论:老年骨质疏松性胸腰椎骨折治疗中联合应用骨水泥成形术与鲑鱼降钙素可提升治疗有效性,减轻术后疼痛度,增强患者的骨密度,加快其腰椎功能的恢复以及骨折愈合情况.
Objective:To observe the effect of bone cement plasty combined with salmon calcitonin in the treatment of senile osteoporotic thoracolumbar fractures.Methods:60 elderly patients with osteoporotic thoracolumbar fractures admitted to our hospital from January 2019 to October 2022 were selected as the research objects.According to the sequence coding method,they were divided into the experimental group and the reference group,with 30 cases in each group.Both groups were treated with bone cement plasty.The Oswestry Disability Index(ODI),Visual Analogue Scale(VAS)score,fracture healing,ambulation,pain relief time,and clinical effect were compared between the two groups.The recovery of injured vertebrae and the improvement of bone mineral density were observed.Results:At 3 and 6 months after operation,the ODI scores of the trial group were lower than those of the reference group,and the difference between the two groups was statistically significant(P<0.05).At 1 week and 1 month after operation,the VAS scores of the trial group were lower than those of the reference group,and the differences between the two groups were statistically significant(P<0.05).The time of fracture healing,ambulation,and pain relief in the trial group were shorter than those in the reference group,and the differences between the two groups were significant(P<0.05).Before treatment,there was no significant difference in Cobb Angle of kyphosis,wedge Angle of injured vertebra,anterior height of injured vertebra,bone mineral density of right femoral neck,and lumbar spine between 2 groups(P>0.05).After treatment,the Cobb Angle of kyphosis and wedge Angle of injured vertebra in the two groups were smaller than those before treatment,and the anterior height of injured vertebra,bone mineral density of right femoral neck and lumbar vertebra were higher than those before treatment,and the Cobb Angle of kyphosis and wedge Angle of injured vertebra in the trial group were smaller than those in the reference group,and the anterior height of injured vertebra,bone mineral density of right femoral neck and lumbar vertebra in the trial group were higher than those in the reference group.The difference was statistically significant(P<0.05).The total effective rate of the experimental group was 96.67%,which was higher than 80.00%of the reference group,and the difference was statistically significant(P<0.05).Conclusion:The combined application of bone cement plasty and salmon calcitonin in the treatment of elderly patients with osteoporotic thoracolumbar fractures can improve the effectiveness of treatment,reduce postoperative pain,enhance bone mineral density,and accelerate the recovery of lumbar function and fracture healing.
