低分子肝素钙在股骨粗隆间骨折内固定患者中的应用OA
Application of Low Molecular Weight Heparin Calcium in Internal Fixation for Patients with Intertrochanteric Fractures of the Femur
目的:探讨在股骨粗隆间骨折患者内固定术中应用低分子肝素钙的效果.方法:选取 2021年 1月—2022年 10月我院收治的 76例股骨粗隆间骨折患者为研究对象,按随机数字表法将其分为对照组和研究组,各 38例.2组患者均行内固定术,对照组采用气压治疗,研究组采用低分子肝素钙治疗,2组均连续治疗 7 d.对比 2组的临床疗效、凝血功能指标、静脉血流速度、下肢周径、手术时间、术中出血量、总出血量、隐性失血量、下肢静脉血栓发生率和不良反应发生率.结果:研究组总有效率高于对照组,差异有统计学意义(P<0.05).术后 7d,研究组PT、APTT长于对照组,D-D水平低于对照组,组间差异有统计学意义(P<0.05).术后 7 d,研究组静脉血流速度高于对照组,下肢周径小于对照组,组间差异有统计学意义(P<0.05).研究组下肢静脉血栓发生率低于对照组,差异有统计学意义(P<0.05).2组均未出现严重不良反应.结论:将低分子肝素钙应用于股骨粗隆间骨折内固定术中可改善患者的凝血功能,可加速血液流动,有利于预防下肢静脉血栓的发生,且患者出血风险较小,具有较高的用药安全性.
Objective:To explore the effect of using low molecular weight heparin calcium in internal fixation surgery for patients with intertrochanteric fractures of the femur.Methods:76 patients with intertrochanteric fractures of the femur admitted to our hospital from January 2021 to October 2022 were selected as the research subjects,divide them into a control group and a research group using a random number table method,with 38 cases.Both groups of patients underwent internal fixation surgery.The control group received pneumatic therapy,while the study group received low molecular weight heparin calcium therapy.Both groups received continuous treatment for 7 days.The clinical efficacy,coagulation function indicators,venous blood flow velocity,lower limb circumference,surgical time,intraoperative bleeding volume,total bleeding volume,occult blood loss,incidence of lower limb venous thrombosis,and incidence of adverse reactions between two groups were compared.Results:The total effective rate of the research group is higher than that of the control group,the difference was statistically significant(P<0.05).7 days after surgery,the PT and APTT in the research group were longer than those in the control group,and the D-D level was lower than that in the control group,the differences were statistically significant between the groups(P<0.05).7 days after surgery,the venous blood flow velocity in the research group was higher than that in the control group,and the circumference of the lower limbs was smaller than that in the control group,the differences were statistically significant between the groups(P<0.05).The incidence of lower limb venous thrombosis in the research group was lower than that in the control group,the difference was statistically significant(P<0.05).No serious adverse reactions were observed in both groups.Conclusion:The application of low molecular weight heparin calcium in internal fixation surgery for intertrochanteric fractures of the femur can improve the patient's coagulation function,accelerate venous blood flow,and prevent the occurrence of lower limb venous thrombosis.Moreover,the patient has a lower risk of bleeding and higher medication safety,which can be promoted and applied in clinical practice.
