基于MRI的多参数影像组学构建预测前列腺癌盆腔淋巴结转移风险预警模型OACSTPCD
Constructing a Multi Parameter Imaging Omics Model Based on MRI for Predicting the Risk of Pelvic Lymph Node Metastasis in Prostate Cancer
目的:分析MRI的多参数影像组学用于前列腺癌(PCa)盆腔淋巴结转移预测价值.方法:选取2018年3月至2023年3月于我院就诊经病理证实的131例PCa患者,分为训练组和验证组,术前均行动态对比增强MRI(DCE-MRI)、前列腺磁共振成像(MRI)平扫及弥散加权成像(DWI)检查.训练组共91例患者,根据患者是否发生盆腔淋巴结转移,分为转移组(n=34)和非转移组(n=57).收集患者的临床及影像学参数,进行单因素及多因素Logistic回归分析,以确定独立预测因素.采用ROC曲线评价预测模型的效能.结果:单因素分析显示,PSA、PI-RADS评分、Gleason评分、ADC值、淋巴信号及DCE图像在两组间存在显著差异(P<0.05).多因素分析确定ADC值、淋巴信号、DCE图像、PI-RADS评分及Gleason评分为独立预测因素(OR值分别为0.014、9.507、21.646、8.965、4.314,均P<0.05).建立的预测模型在训练组中ROC曲线下面积为0.936(95%CI:0.890~0.982),灵敏度为97.1%,特异性为77.8%,准确度为87.5%;在验证组中曲线下面积为0.929(95%CI:0.867~0.990),灵敏度为88.2%,特异性为93.0%,准确度为90.6%.结论:ADC值、淋巴信号、DCE图像、PI-RADS评分及Gleason评分是预测PCa患者盆腔淋巴结转移的独立因素,基于此建立的风险预警模型具有良好的预测效能.
Objective:To analyze the value of MRI multiparameter imaging in predicting pelvic lymph node metastasis in prostate cancer(PCa).Methods:A total of 131 pathologically confirmed PCa patients admitted to our hospital from March 2018 to March 2023 were selected and divided into training group and validation group.Dynamic contrast enhanced MRI(DCE-MRI),prostate magnetic resonance imaging(MRI)plain scan and diffusion-weighted imaging(DWI)were performed before surgery.A total of 91 patients in the training group were divided into metastatic group(n=34)and non-metastatic group(n=57)according to whether they had pelvic lymph node metastasis.Clinical and imaging parameters of patients were collected and univariate and multivariate Logistic regression analyses were performed to determine independent predictors.ROC curve was used to evaluate the efficacy of the prediction model.Results:Univariate analysis showed that PSA,PI-RADS score,Gleason score,ADC value,lymph signal and DCE image were significantly different between the two groups(P<0.05).Multivariate analysis determined that ADC value,lymphatic signal,DCE image,PI-RADS score and Gleason score were independent predictors(OR values were 0.014,9.507,21.646,8.965,4.314,respectively,all P<0.05).The area under ROC curve of the established model in the training group was 0.936(95%CI:0.890-0.982),the sensitivity was 97.1%,the specificity was 77.8%,and the accuracy was 87.5%.In the validation group,the area was 0.929(95%CI:0.867-0.990),the sensitivity was 88.2%,the specificity was 93.0%,and the accuracy was 90.6%.Conclusion:ADC value,lymphatic signal,DCE image,PI-RADS score and Gleason score are independent factors for predicting pelvic lymph node metastasis in PCa patients,and the risk warning model established based on these factors has good predictive efficacy.
