基于超声成像技术鉴别子宫黏膜下肌瘤与非典型息肉样腺肌瘤OACSTPCD

Objective:To explore the differential diagnosis of submucosal fibroids and atypical polypoid adenomyomas based on clinical features and ultrasound imaging,in order to reduce the occurrence of misdiagnosis.Methods:The imaging data of 85 patients with submucosal fibroids and 68 patients with atypical polypoid adenomyoma who underwent routine ultrasound examination and were pathologically confirmed in our hospital from September 2021 to March 2023 were retrospectively analyzed.The baseline data were matched by propensity matching method,and the Rad score values of the two groups of patients were calculated and analyzed by Wilcoxon test.Results:Before and after propensity matching,there was statistical significance in isoechoic,single-vessel pattern and annular vessel pattern between the two groups(P＜0.05).Taking pathological examination as the"gold standard",78 cases of uterine submucosal fibroids were detected by ultrasound,the coincidence rate was 91.76％.There were 62 cases with atypical polypoid adenomyoma,the coincidence rate was 91.18％.The comparison of misdiagnosis was statistically significant(P＜0.05).The mean Rad score of uterine submucosal myoma and atypical polypoid adenomyoma were 5.231 and 4.628,respectively(P＜0.05).Conclusion:Ultrasound imaging technology can effectively differentiate uterine submucosal fibroids from atypical polypoid adenomyomas,among which equal-echo,single-vessel pattern,annular vessel pattern and fibroid volume are more effective.
