外周血炎症标志物对视网膜静脉阻塞性黄斑水肿的预测价值OACSTPCD
Predictive value of peripheral blood inflammatory markers in retinal vein occlusion-related macular edema
目的 探讨外周血炎症标志物对视网膜静脉阻塞性黄斑水肿(RVO-ME)发生的预测价值,寻找预测RVO-ME发生的炎症生物标志物.方法 回顾性选取2020年8月至2023年12月就诊于该院眼科的87例RVO-ME患者作为病例组,并选取同期白内障手术患者87例作为对照组.根据阻塞位置的不同将病例组患者分为视网膜中央静脉阻塞(CRVO)亚组、视网膜分支静脉阻塞(BRVO)亚组.对各组外周血血小板与淋巴细胞比值(PLR)、单核细胞与淋巴细胞比值(MLR)、中性粒细胞与淋巴细胞比值(NLR)进行比较,并采用受试者工作特征(ROC)曲线评价PLR、MLR、NLR对RVO-ME的预测价值.结果 病例组外周血PLR、MLR、NLR均高于对照组,差异均有统计学意义(P<0.05),BRVO亚组与CRVO亚组间PLR、MLR、NLR比较,差异均无统计学意义(P>0.05).ROC曲线分析结果显示,MLR(曲线下面积为0.727,95%CI:0.651~0.803)对RVO-ME的发生具有较好的预测价值.结论 外周血炎症标志物PLR、MLR、NLR与RVO-ME的发生密切相关,MLR对RVO-ME发生具有较好的预测价值,或可成为预测RVO-ME发生的炎症生物标志物.
Objective To investigate the predictive value of peripheral blood inflammatory markers on the occurrence of retinal vein occlusive macular edema(RVO-ME),and to find the inflammatory biomarkers for predicting the occurrence of RVO-ME.Methods A total of 87 patients with RVO-ME visiting in the ophthal-mology department of this hospital from August 2020 to December 2023 served as the case group,and 87 pa-tients undergoing cataract surgery during the same period were selected as the control group.The patients in the case group were divided into the CRVO subgroup and BVRO subgroup according to the different occlusion positions.The peripheral blood platelet to lymphocyte ratio(PLR),monocyte to lymphocyte ratio(MLR)and neutrophils to lymphocytes ratio(NLR)were compared between the two groups,and the receiver operating characteristic(ROC)curve was used to evaluate the predictive value of peripheral blood PLR,MLR,NLR for the occurrence of RVO-ME.Results The peripheral blood PLR,MLR and NLR in the case group were higher than those in the control group,and the differences were statistically significant(P<0.05).But there were no statistically significant differences in PLR,MLR and NLR between the BRVO subgroup and CRVO subgroup.The ROC curve analysis results showed that MLR[the area under the curve(AUC)=0.727,95%CI:0.651-0.803)]had better predictive value for RVO-ME occurrence.Conclusion The peripheral blood inflammatory markers PLR,MLR and NLR are closely correlated to the occurrence of RVO-ME.MLR had good predictive value for RVO-ME occurrence and may become as a inflammation biomarker to predict the occurrence RVO-ME.
