Relationship between p38 MAPK mRNA and Sirt1 mRNA levels in tears with Conbercept therapeutic effect in patients with polypoidal choroidal vasculopathy

Abstract
Objective To investigate the relationship between the levels of p38 mitogen-activated protein kinase(MAPK)mRNA and silencing information regulatory factor 2 related enzyme 1(Sirt1)mRNA with the Conbercept therapeutic effect in the patients with polypoidal choroidal vasculopathy(PCV).Methods A total of 94 patients with PCV in this hospital from August 2020 to April 2022 were selected as the observation group and divided into the good effect subgroup and poor effect subgroup according to the Conbercept thera-peutic effect.In the same period,94 age and gender matched healthy subjects undergoing physical examination served as the control group.The levels of tear p38 MAPK mRNA,Sirt1 mRNA and vascular endothelial growth factor(VEGF)were detected in both groups.The correlation between tear p38 MAPK mRNA and Sirt1 mRNA with VEGF was analyzed.The receiver operating characteristic(ROC)curve was used to analyze the predictive value of p38 MAPK mRNA,Sirt1 mRNA and VEGF levels in the Conbercept therapeutic effect.The multivariate Logistic regression was used to analyze the influencing factors of the Conbercept ther-apeutic effect.Results The levels of tear p38 MAPK mRNA and VEGF before treatment and after 12-week treatment in the observation group were higher than those in the control group,the Sirt1 mRNA level was lower than that in the control group,and the differences were statistically significant(P＜0.05).The levels of tear p38 MAPK mRNA and VEGF after 12-week treatment in the observation group were lower than those before treatment,and the Sirt1 mRNA level was higher than that before treatment,and the differences were statistically significant(P＜0.05).The p38 MAPK mRNA levels before treatment and after 12-week treat-ment were positively correlated with VEGF(r=0.717,0.338,P＜0.001)and the Sirt1 mRNA levels before treatment and after 12-week treatment were negatively correlated with VEGF(r=-0.766,-0.396,P＜0.001).Ninety-four patients with PCV were followed up for 1 year,and the complete subsided rate of polyps was 40.66％.The levels of p38 MAPK mRNA and VEGF before treatment and after 12-week treatment in the poor effect subgroup were higher than those in the good effect subgroup,and the Sirt1 mRNA level was lower than that in the good effect subgroup,and the differences were statistically significant(P＜0.05).The area under the curre(AUC)of tear p38 MAPK mRNA,Sirt1 mRNA and VEGF levels combination after 12-week treatment for predicting the Conbercept therapeutic effect in the patients with PCV was greater than that of single index(P＜0.05).Both p38 MAPK mRNA and Sirt1 mRNA were the influencing factors of the Conber-cept therapeutic effect in the patients with PCV(P＜0.05).Conclusion The expression of p38 MAPK mR-NA and Sirt1 mRNA in the patients with PCV is abnormal and both are related to the Conbercept therapeutic effect.关键词
