目的 分析不同的甲状腺穿刺针联合不同细胞学制片方法对甲状腺结节细针穿刺取材效果的影响,寻找甲状腺细针穿刺细胞学检查的最佳模式.方法 选择2020年6月至2021年8月在安徽医科大学附属滁州医院行超声引导下甲状腺细针穿刺细胞学检查的240例患者,分为4个组:注射器穿刺+常规涂片细胞学检查组、注射器穿刺+液基细胞学检查组、吸引活检针穿刺+常规涂片细胞学检查组、吸引活检针穿刺+液基细胞学检查组,比较4组患者细胞学诊断报告中不能诊断/不满意(取材失败)发生率以及穿刺并发症、穿刺疼痛程度.结果 (1)4组间取材结果差异有统计学意义(P<0.05);进一步分析显示,注射器+液基的取材失败率较注射器+常规涂片组低(P<0.05),其他组别之间取材失败率差异无统计学意义.(2)注射器穿刺组和吸引活检针穿刺组取材结果差异无统计学意义(x2=0.144,P=0.704);常规涂片组取材失败率高于液基组(x2=5.192,P=0.023).(3)按照结节弹性评分分层分析:当弹性评分2分时,注射器穿刺组取材失败率高于吸引活检针穿刺组(x2=5.103,P=0.024);当弹性评分>2分时,2种穿刺针取材失败率差异无统计学意义(x2=1.305,P=0.254).(4)按照结节血流强度分层分析:当血流强度>1级时,常规涂片组取材失败率高于液基细胞学组(P=0.006).(5)4组间疼痛评分差异有统计学意义(P=0.015),注射器穿刺组疼痛评分高于吸引活检针穿刺组(x2=3.165,P=0.002).(6)穿刺并发症情况:注射器+常规涂片组发生1例局部血肿和1例低血压,吸引活检针+常规涂片组发生1例低血压,其余2组无并发症发生.结论 注射器和吸引活检针取材结果整体上无差异.根据甲状腺结节的超声报告选择穿刺及制片方法可提高成功率.

Objective To analyse the effects of different thyroid puncture needles combined with different cyto-logical preparation methods on the effects of fine needle aspiration sampling of thyroid nodules,and search for the opti-mal mode of fine needle aspiration cytology of the thyroid.Methods Two hundred and forty patients who underwent ul-trasound-guided thyroid fine-needle aspiration cytology from June 2020 to August 2021 at the Affiliated Chuzhou Hospital of Anhui Medical University were selected.They were divided into 4 groups:syringe puncture+conventional smear cytol-ogy examination group,syringe puncture+liquid-based cytology examination group,suction biopsy needle puncture+conventional smear cytology examination group,and suction biopsy needle puncture+liquid-based cytology examination group,and compared the incidence of undiagnostic/unsatisfactory(sampling failure)in the cytological diagnostic reports of the patients among 4 groups,as well as the degree of puncture complications and puncture pain.Results(1)The difference in sampling results among 4 groups was statistically significant(P<0.05);further analysis showed that the rate of sampling failure was lower in the syringe+liquid-based than in the syringe+routine smear group(P<0.05),and the difference in the rate of sampling failure between the other groups was not statistically significant.(2)There was no statistically significant difference in the extraction results between the syringe group and the attraction biopsy needle group(x2=0.144,P=0.704);the extraction failure rate of the conventional smear group was higher than that of the liquid-based group(x2=5.192,P=0.023).(3)Stratified analysis according to nodal elasticity score:when the elastic-ity score=2,the failure rate of sampling in the syringe group was higher than that in the suction biopsy group(x2=5.10,P=0.024);when the elasticity score was>2,the difference in the failure rate of sampling between the 2 types of perforating needles was not statistically significant(x2=1.3,P=0.254).(4)Stratified analysis according to nodal blood flow intensity:when blood flow intensity>1,the routine smear group had a higher sampling failure rate than the liquid-based cytology group(P=0.006).(5)The difference in pain scores among the 4 groups was statistically signifi-cant(P=0.015),and analysis of the combined same puncture needle groups showed that the pain score in the syringe group was higher than that in the suction biopsy needle group(x2=3.165,P=0.002).(6)Complications of puncture:1 case of local haematoma and 1 case of hypotension occurred in the syringe+routine smear group,1 case of hypotension occurred in the attraction biopsy needle+routine smear group,and no complications occurred in the remaining 2 groups.Conclusions There is no overall difference in the sampling results between the syringe and the suction biopsy needle.Se-lecting puncture and sectioning methods based on the ultrasound report of thyroid nodules can improve the success rate.
