目的 探讨数字断层融合摄影(DTS)在胫骨骨折内固定术后复查中的应用价值.方法 回顾性分析2020年10 月至2023 年10 月在我院行胫骨骨折内固定术后的患者120 例,于术后3 个月及6 个月时行数字化X线摄影(DR)及DTS检查,比较两种影像学方法的图像质量和骨折愈合显示情况,并对2 名医师的图像质量评分结果进行一致性分析.结果 2 名医师在DTS及DR的图像质量评分结果上均有较好的一致性.术后 3 个月时,DTS图像质量评分为(3.658±1.170)分,DR图像质量评分为(2.750±1.176)分,两组差异有统计学意义(P<0.05).DTS图像骨折愈合情况显示率为91.67%(110/120),DR图像骨折愈合情况显示率为 72.50%(87/120),差异有统计学意义(P<0.05).术后 6 个月时,DTS图像质量评分为(3.692±1.165)分,DR图像质量评分为(2.792±1.187)分,两组差异有统计学意义(P<0.05).DTS图像骨折愈合情况显示率为93.33%(112/120),DR图像骨折愈合情况显示率为75.83%(91/120),差异有统计学意义(P<0.05).结论 在胫骨骨折内固定术后复查中,DTS图像质量优于DR,能更加清晰地显示术区骨质结构及骨折愈合情况,有助于临床医师更准确地评估内固定术后恢复情况,具有较好的临床应用价值.

Objective To investigate the application value of digital tomosynthesis(DTS)in the review of tibial fracture after internal fixation.Methods A total of 120 cases of tibial fracture patients who underwent fixation surgery in our hospital from October 2020 to October 2023 were examined by digital radiography(DR)and DTS at 3 and 6 months after operation.The image quality and fracture healing display of the two imaging methods were compared,and the consistency of the image quality scores of the two doctors were evaluated.Results The two doctors showed good consistency in the image quality scores of DTS and DR.At 3 months after operation,the DTS image quality score was(3.658±1.170)and the DR image quality score was(2.750±1.176),with a statistically significant difference between the two groups(P<0.05).The display rate of fracture healing on DTS images was 91.67%(110/120),while the display rate of fracture healing on DR images was 72.50%(87/120),with a statistically significant difference(P<0.05).At 6 months after operation,the DTS image quality score was(3.692±1.165)and the DR image quality score was(2.792±1.187),with a statistically significant difference between the two groups(P<0.05).The display rate of fracture healing on DTS images was 93.33%(112/120),while the display rate of fracture healing on DR images was 75.83%(91/120),with a statistically significant difference(P<0.05).Conclusion In the follow-up examination of tibial fractures after internal fixation,the image quality of DTS is better than that of DR,which can more clearly display the bone structure and fracture healing situation in the operative area.It helps clinicians to evaluate the recovery after internal fixation more accurately and has a high clinical application prospect.
