老年冠心病患者血清脂联素、生长素释放肽水平与血小板聚集率及主要不良心脏事件相关性分析OACSTPCD
Correlation analysis of serum APN and Ghrelin levels with platelet aggregation rate and major adverse cardiac events in elderly patients with coronary heart disease
目的:探讨老年冠心病(CHD)患者血清脂联素(APN)、生长素释放肽(Ghrelin)水平与血小板聚集率(PAG)及主要心脏不良事件(MACE)之间的相关性.方法:选择83例老年CHD患者为病例组,另收集同期72例健康体检者作为对照组.入院后收集所有受试者一般资料,并检测其空腹血糖、血脂等生化指标.酶联免疫吸附法检测血清APN、Ghrelin水平.采用血小板聚集仪比浊法测定PAG,分析其与血清APN、Ghrelin水平之间的相关性.出院后随访12周,记录患者MACE发生情况,分析血清APN、Ghrelin水平与MACE之间的相关性.结果:CHD组与对照组总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、血尿酸(UA)、左心室舒张末期内径(LVEDD)及左心室射血分数(LVEF)等水平比较,差异有统计学意义(均P<0.05).与对照组比较,CHD组APN与Ghrelin水平明显降低(均P<0.05).与对照组比较,CHD组PAG显著增加(均P<0.05).Pearson相关性分析显示,CHD患者血清APN、Ghrelin水平与PAG呈明显负相关(r=-0.792,0.578,均P<0.05).83例CHD患者有26例发生MACE,与未发生MACE患者比较,两组HDL、LVEDD、LVEF、APN及Ghrelin水平比较差异有统计学意义(均P<0.05).Logistic回归分析显示,HDL、LVEF、APN、Ghrelin是MACE发生的保护因素(均P<0.05).结论:老年CHD患者血清APN、Ghrelin水平显著降低,且与血小板聚集率及预后有着密切关系,低水平APN、Ghrelin可一定程度上反映CHD患者MACE的发生风险.
Objective:To investigate the correlation between serum adiponectin(APN),Ghrelin levels,platelet aggregation rate(PAG)and major adverse cardiac events(MACE)in elderly patients with coronary heart disease(CHD).Methods:A total of 83 elderly patients with CHD were selected as the case group,and 72 healthy subjects were selected as the control group.After admission,the general data of all subjects were collected,and their fasting blood glucose,blood lipid and other biochemical indicators were detected.Serum APN and Ghrelin levels were detec-ted by enzyme-linked immunosorbent assay.The correlation between PAG and serum APN、Ghrelin levels was ana-lyzed by turbidimetry with platelet aggregator.The patients were followed up for 12 weeks after discharge,the occur-rence of MACE was recorded,and the correlation between serum APN、Ghrelin level and MACE was analyzed.Results:There were significant differences in total cholesterol(TC),triglyceride(TG),high density lipoprotein(HDL),low density lipoprotein(LDL),blood uric acid(UA),left ventricular end-diastolic inner diameter(LVEDD)and left ventricular ejection fraction(LVEF)between CHD group and control group(all P<0.05).Compared with the control group,the levels of APN and Ghrelin in CHD group were significantly decreased(all P<0.05).Com-pared with the control group,PAG in CHD group was significantly increased(P<0.05).Pearson correlation analysis showed that serum APN and Ghrelin levels were significantly negatively correlated with PAG in CHD patients(r=-0.792,-0.578,all P<0.05).Among the 83 CHD patients,26 had MACE.Compared with the patients without MACE,there were significant differences in HDL、LVEDD、LVEF、APN and Ghrelin between the two groups(all P<0.05).Logistic regression analysis showed that HDL、LVEF、APN and Ghrelin were protective factors for MACE occurrence(all P<0.05).Conclusion:The serum levels of APN and Ghrelin levels in elderly CHD patients are significantly reduced,and are closely related to platelet aggregation rate and prognosis.Low levels of APN and Ghrelin can reflect the risk of MACE in CHD patients to a certain extent.
